Description

PRE-QUALIFICATIONS - Bidders shall be pre-qualified by the Michigan Department of Transportation or have available, upon request, three references of previous work experience including the company name, address, phone, and contact person. SCOPE OF WORK - This project includes removing existing pavement, excavation, underdrain, structure taps, drainage course, HMA paving, and aggregate shoulders and HMA curb as directed by the BCRC / MDOT. BID REQUIREMENTS - The undersigned has examined the location(s) of the work described herein and is fully informed as to the nature of the work and conditions relating to its performance and understands the quantities shown are approximate and are subject to either increase or decrease. The undersigned hereby proposes to furnish all necessary equipment, tools, apparatus, and other means of construction, do all of the work, furnish all materials except as otherwise specified herein; and, for the unit prices named in the attached Itemized Bid Form, to complete the work herein described in strict accordance with the plans and the requirements of these bid documents. The undersigned further proposes to perform extra work (for items that are not included with the itemized form) that may be authorized by the BCRC. Compensation for extra work will be made on the basis of an agreed upon unit price prior to performing the extra work. BID WITHDRAWAL - No bid may be withdrawn for a period of 60 calendar days after bid opening. INSURANCE REQUIREMENTS - The contractor shall at all times exercise extreme care and shall hold the Benzie County Road Commission harmless from any claims or damages. The successful bidder shall provide proof of insurance for General Liability, Auto Liability, and Worker's Compensation and shall name Benzie County Road Commission as an additional insured for general liability. Certificates shall be on file with the Road Commission prior to the start of any work. CONSTRUCTION SCHEDULE - The contractor shall provide a construction schedule upon request. Work shall be coordinated with the Road Commission's preparation schedule. All work shall be confined to daylight hours. No work shall be performed on holidays, as determined by the BCRC. The contractor shall give the BCRC Superintendent or Inspector twenty-four (24) hours' notice prior to beginning work at any location. TAXES - The contractor shall pay all Michigan Sales and Use taxes. SUBCONTRACTING - The approval of the Road Commission shall be obtained prior to any substantial work being subcontracted. The contractor shall provide proof that any subcontractor has sufficient equipment, insurance and personnel to properly complete any item of work within the specified time. START DATE - All projects will be ready to begin in accordance with the Michigan Department of Transportation Construction Manual, and weather dependent. Unless noted in the special notes.
