Description | PURPOSE: The Virginia Department of Transportation (herein referred to as "VDOT") is soliciting bids from qualified firms to provide horizontal concrete sawing services to alleviate trip/fall hazards on walkways in Accomack and Northampton Counties (Eastern Shore). PERIOD OF CONTRACT: From Start Date specified on the Notice of Award through One (1) year. (Renewable) QUESTIONS REGARDING THIS INVITATION FOR BIDS: Any questions regarding this Invitation for Bids shall be addressed to Rhonda Tharrington at rhonda.tharrington@VDOT.virginia.gov. The issuing office shall determine whether any addendum should be issued as a result of any questions or other matters raised. Provide questions no later than five (5) business day prior to bid closing date. SCOPE OF WORK: The Contractor shall provide all equipment with qualified, experienced operator(s), laborer(s) fuel, tools, parts, supplies, insurance, and incidentals as necessary for the safe and efficient operation of the equipment required to perform concrete sidewalk repairs, defined as removing trip and fall hazards on concrete sidewalks by cutting of concrete surfaces within the Accomac Residency (Accomack and Northampton Counties). Quantities are estimated for what may be needed over the course of the year however, quantities ordered and paid for will be based on actual need. A change order will be issued if needed quantities exceed those on the initial PO. The Contractor shall: 1. Establish and provide an initial list of all proposed trip/fall hazards to VDOT Contract Administrator (or designee) within ninety (90) days of effective start date on the Notice of Award and prior to any work being performed based on direction provided by VDOT. The detailed list shall include: a. The exact location of proposed repair (street name, address, GPS location) b. The hazard identified and categorized by severity as listed below: Less Severe: .25" - .5" in Height Severe: .5" -- 1" in Height Most Severe: l" 2.5" in Height |
