This is a regional contract that includes counties within District Six. ***Proposal Schedule of Items for Emergency Lighting Permanent Repair and Replacement MUST be submitted with your bid as an attachment. FHWA 1273 and Davis Bacon Wage Rates are applicable for this Contract. The Contractor shall provide a letter to the Department at the time of bid, from a surety authorized to do business in the State of Florida, verifying the Contractor is bondable in the State of Florida. For the purposes of this Contract the dollar amount is established at $500,000.00. All Lighting Repair and Replacement Operations performed under this Contract must be in accordance with the Florida Department of Transportation Standard Specifications for Road and Bridge Construction, any Specifications detailed in this scope, and Standard Plans. Have a minimum of ten (10) Lighting Repair and Replacement Crews available for reporting directly to work sites. The minimum number of Lighting Repair and Replacement Crews activated must remain available until officially released by the Department. No quantities are guaranteed and there will be no unit cost adjustment for quantities required over or under the estimated quantities. This contract will be awarded based on the total amount of $0 zero dollars and not the contractor's bid total amount. Additional funds will be added to the contract as needed.. Pre-qualification is Required to Bid on This Project. Proposal Guaranty of $500 is required for bids over $150,000 and may be in the form of a Cashier's Check, the BDI Bid or Proposal Bond (Form No. 375-020-16), or the standard Bid or Proposal Bond form, Form No. 375-020-09. For bids over $150,000.00, the standard proposal guaranty of 5% of the bid will be required, unless otherwise stipulated in the proposal advertisement. A Proposal Guaranty of not less than five percent (5%) of the total actual bid in the form of either a certified check, cashier's check, trust company treasurer's check, bank draft of any national or state bank, or a Surety Proposal Guaranty made payable to the Florida Department of Transportation must be received for each bid in excess of $150,000.00. A 100% Payment and Performance Bond will be required for all projects unless noted otherwise in the project specifications.
