Description

To provide all personnel and labor, materials, vendors, equipment, etc, to provide biohazardous waste transportation and disposal. Pricing is to include the following elements: 1. Assist the University in preparing waste for pick up and shipment by providing required labels, packaging instructions, shipping manifests, containers, documents and any other relative information needed. Transport from the University designated pick up locations to the approved licensed disposal facility. 2. Provide proper OSHA and DOT approved biohazardous waste containers in various sizes eg. small, medium and large eg. 20, 44 and 55 gallons including lids for onsite lab accumulation, storage and off site shipping. If containers are reusable, they must be cleaned and disinfected prior to being returned. 3. Provide red biohazard liners for biohazard waste and non-sharps containers in corresponding sizes for each container delivered to EHS. 4. Dispose of various sizes of full disposable sharps containers. If sharps containers are reusable, containers will be returned cleaned and disinfected for reuse. 5. Stock EHS with necessary amount of each size of biohazard container at each waste pick up location and deliver extra containers and liners when needed. 6. Provide training to EHS staff and all other University location personnel as necessary in the safe handling, transportation and management techniques required to properly accommodate the services outlined in this RFP. 7. Maintain complete tracking of all biohazardous waste at all times while in transit. 8. Provide emergency notification capability to meet DOT requirements 9. Identify suitable licensed and permitted facilities for treatment and disposal of biological wastes, and transport packaged biological wastes to those facilities within prescribed time limits. 10. Furnish to the University proof of disposal and/or certificates of destruction with each shipment. 11. Provide all waste acceptance protocols. 12. Provide all legal licenses and permits for disposal contractor and disposal facility and transportation assets. 13. Provide a detailed explanation of biohazardous waste tracking systems and protocols from collection to disposal. 14. Furnish to the University Director of Environmental Health and Safety or authorized representative appropriate documentation, such as respondents proof of receipt of shipment or signed manifests, showing proper disposition of all waste. 15. All activities of the supplier shall be fully documented and coordinated with the University. The University shall have access to all such documentation at any time.
