Description

Scope of Services The City of Ontario is requesting proposals from qualified firms to provide preliminary design alternates and a feasibility analysis to widen a three-mile stretch of Edison Avenue/Ontario Ranch Road from two lanes to four lanes from Euclid Avenue to Cucamonga Creek Channel (west of Archibald Avenue). The overall project design will be executed in two (2) phases. Phase I will be preliminary engineering, including utility and right-of-way research and environmental determination, and the second phase will be the final design, environmental clearance and right of way support phase, which will require services through the completion of the construction bid package (PS&E). Phase II will be considered as optional scope of work to be exercised by the City at its sole discretion. Other Details Bidder must attach PDF of completed Non-Collusion Declaration form as a "General Attachment." The bidder shall submit an electronic copy of their technical proposal as a PDF in the "General Attachment" field in the "Attachments" tab, an electronic copy of their fee proposal as a PDF in the "Cost File" field of the "Attachments" tab. (***The Cost File is MANDATORY and the Bid Management System will not accept your submission unless a "Cost File" attachment is uploaded.***) One (1) Executed Original and Three (3) Copies of the Technical Proposal and One (1) Copy of the Fee Proposal must also be submitted by the electronic submission deadline. The hard copies must be submitted to the Records Management department, 303 E. "B" Street, Ontario, CA 91764. Notes The bidder is solely responsible for "on time" submission of their electronic proposal. The City will only consider proposals that have been transmitted successfully and have been issued an e bid confirmation number with a time stamp from the Bid Management System indicating that bid was submitted successfully. Transmission of bids by any other means will not be accepted. Bidder shall be solely responsible for informing itself with respect to the proper utilization of the bid management system, for ensuring the capability of their computer system to upload the required documents, and for the stability of their internet service. Failure of the bidder to successfully submit an electronic bid shall be at the bidder's sole risk and no relief will be given for late and/or improperly submitted proposals.
