Description | Public Sewer and Public Water Connection for Failing Onsite Sewage System and Well 3970 Longhill Road Williamsburg, Virginia 23188 Scope of work: Cost for Line Item #1 (include total cost for items A-J) These are known factors. Vendors must invoice for actual cost incurred as described herein. A) Construct a private sewer line connecting the subject property to the James City Service Authority (JCSA) public sewer line in accordance with the Virginia Uniform Statewide Building Code (VUSBC), and all other applicable state and local laws, regulations, and ordinances. Follow Appendix H - Residential Grinder Pump Procedures as required by JCSA to enter into a grinder pump service agreement. B) Construct a private water line connecting the subject property to the James City Service Authority (JCSA) public water line in accordance with the Virginia Uniform Statewide Building Code (VUSBC), and all other applicable state and local laws, regulations, and ordinances. C) Within 48 hours of signature of agreement to construct the sewer line and water line, the contractor shall have the contents of the existing septic tank serving the subject property pumped by a properly licensed sewer hauler. The contents of the existing septic tank shall be continuously pumped every 14 days thereafter, until connection to the JCSA public sewer line is complete. D) Serving as an agent for the property owner, the contractor shall contact JCSA and fill out all necessary applications to initiate JCSAs installation of a sewer tap to the edge of the Virginia Department of Transportations Right of Way at 3970 Longhill Road Williamsburg, Virginia 23188. Initiation of JCSAs sewer tap installation shall include payment of the $5,411.00 connection fee. E) Serving as an agent for the property owner, the contractor shall contact JCSA and fill out all necessary applications to initiate JCSAs installation of a water tap to the edge of the Virginia Department of Transportations Right of Way at 3970 Longhill Road Williamsburg, Virginia 23188. Initiation of JCSAs water tap installation shall include payment of the $5,165.00 connection fee. F) Upon JCSA completion of sewer tap installation, contractor shall install a private sewer line and grinder pump in compliance with the VUSBC and Appendix H Residential Grinder Pump Procedures. The 145-foot private sewer line will begin at the grinder pump and terminate at the JCSA tap. G) Upon JCSA completion of water tap installation, contractor shall install a private water line in compliance with the VUSBC. The 95-foot private sewer line will begin at the front of foundation and terminate at the JCSA tap. H) Abandon any unused component of the former onsite sewage system. The existing septic tank shall be pumped by a properly licensed sewage hauler, the tank crushed in place, lime placed over the crushed tank, and the tank hole filled with clean backfill material. I) Follow all regulations and permitting pertaining to erosion and sediment control including stabilizing, seeding and grading the site after construction to return to the original state. Control construction runoff with proper practices so as not to become a nuisance to the owner or to neighboring properties or cause sediment to be discharged into state waters and drainage ditches. Any construction debris must also be removed from the site and disposed of properly. J) Bidders shall comply with all requirements of the Department of Professional and Occupational Regulations (DPOR) for contracting and executing the contract with the Virginia Department of Health. Must provide a copy of Class A, B, or C Contractors License from DPOR, plumbers licensed from DPOR, proof of insurance. Bidders contacted by VDH to provide any missing required documents, must provide the document within 24 business hours or their bid will be considered non-responsive. Additional Services (If Needed) Bidder must contact VDH Project Manager listed on the Notice to Proceed for approval prior to any additional services performed. VDH will issue a change order for actual additional services rendered. The contractor shall furnish all labor, supervision, equipment, tools, parts, supplies and materials, as necessary, to perform the services as described herein: A. Additional materials B. Additional labor and equipment. Breakdown of Total Cost When responding to solicitation Bidders must attach a document listing breakdown of total cost for line item 1. Bidders must attach a separate breakdown of cost for additional services. Additional cost should not be included in line item 1 bid submission.
