Description | This Statement of Work is for the pick-up, transportation, and disposal of the contents of a 35 yd. capacity Hydraulic Cardboard Compactor from the Wilkes-Barre VA Medical Center, 1111 East End Boulevard, Wilkes-Barre, PA 18711 to a cardboard recycling facility. Pick-up of the cardboard shall be accomplished on call, Monday through Friday (excluding Federal Holidays) with a twenty-four (24) hour notice from the Wilkes-Barre VA Medical Center. On the scheduled pick up day, the Contractor shall report to the Wilkes-Barre VA Medical Center to pick-up the cardboard compactor, transport, and dispose of the contents of a Government Owned 35 yd. capacity Hydraulic Cardboard Compactor. The Contractor shall provide all necessary equipment and labor to load, transport, and dispose of the contents of the hydraulic cardboard compactor. Contractor is responsible for returning the hydraulic cardboard compactor to its proper location and operating condition after unloading the contents. The Contractor is responsible for maintaining a record of the weight of the disposed hydraulic cardboard compactor contents for each haul during the life of the contract. On a monthly basis, the vendor shall provide a report of the weight of the disposed contents for each haul to the Environmental Supervisor. Any equipment used by the Contractor to remove and/or transport cardboard from the Wilkes-Barre VA Medical Center to a recycling facility shall be properly licensed and maintained in a safe operating condition. Any training required for the Contractor's personnel to load, transport and dispose of the contents of the hydraulic cardboard compactor is the responsibility of the Contractor. It is the responsibility of the contractor's personnel to obey facility traffic regulations. Failure to obey facility traffic regulations may result in a VA Police Citation. Information on facility traffic regulations may be obtained from VA Police, the COR or his/her designee. Wilkes-Barre VA Medical Center does not make reimbursement for traffic violations of the contractor's personnel under any circumstance. | ||
