Description | The State of Utah Division of Purchasing is the issuing procurement unit and State of Utah Division of Oil, Gas and Mining_ is the conducting procurement unit for this IFB (referred to as the State). The reference number for this IFB is Solicitation #JH22-19. This solicitation number must be referred to on all bids, correspondence, and documentation submitted to the State relating to this IFB. The purpose of this IFB is to enter into a contract with the lowest responsive and responsible bidder to provide: services according to Attachment B Scope of Work This contract will result in one purchase order award to the lowest cost responsive and responsible bidder. The contract resulting from this IFB will be for FIVE (5) years. Each bid received shall be evaluated for responsiveness in as outlined in the Utah Procurement Code. This IFB shall be evaluated by the objective criteria described in this IFB. Bids submitted must comply with the prerequisites and questions sections of this IFB. Any bid that does not comply with the prerequisite and questions sections will be deemed non-responsive and will be rejected. Question Submission Close Date 9/15/2022 2:00 PM MDT The work to be done shall consist of furnishing all labor, materials, and equipment for the complete and satisfactory permanent abandonment of one water monitoring well. The CONTRACTOR shall comply with all federal, state, and local laws and ordinances relating to performances of the work. All work is to be done in accordance with Utah Administrative Code Title R655-4-14. However, due to the location and unique condition of this well the plugging method may need to deviate from standard convention, and a variance obtained from Utah Division of Water Rights. The well is located approximately one-half mile from the paved Consumers Road southwest of Helper, Utah. Due to the rugged topography that surrounds the site the well may only be accessed on foot. Additionally, the bottom of the well was permanently damaged by coal mining activities during the time the Horizon Mine was active. Consequently, the bottom of the well bisects and empties into the underground mine workings. |
