Description | City of Clovis that general contractors and mechanical, electrical and plumbing subcontractors planning to participate in bidding on t h e City's projects funded by the State of California ANNUALLY will be required to prequalify prior to bidding. Prequalification Application packages are available on the City's website. The City of Clovis reserves the right to accept prequalification packets throughout the 2023 calendar year Pre-Bid Meeting No Type of Award Lump Sum Online Q&A No Each prospective respondent shall use this Prequalification Questionnaire and complete each question as directed below. "Contractor", "You", and "Your" is used in this Prequalification Questionnaire to refer to the respondent's firm and any of its owners, officers, principals, and qualifying individuals. Additional page(s) may be attached if space is needed to respond to any questions. The completed Prequalification Questionnaire and financial statements submitted therewith shall not be public records and shall not be open to public inspection. All questions in the questionnaire must be answered. If a question is not applicable, then indicate a response of "N/A". "You" or "Yours" as used herein refers to the prospective bidders' firm and any of its owners, officers, principals and qualifying individuals. Each questionnaire must be signed under penalty of perjury in the manner designated at the end of the form, by an individual who has the legal authority to bind the contractor on whose behalf that person is signing. If any information provided by a contractor becomes inaccurate, the contractor must immediately notify the City and provide updated accurate information in writing, under penalty of perjury. Each prospective bidder shall have a duly authorized owner, officer or principal complete the questionnaire and verify the truth of the information provided therein and in the financial statement. The City reserves the right to amend this Prequalification Application at any time through the issuance of one or more addenda. The City reserves the right to waive minor irregularities and omissions in the information contained in a Prequalification Application that has been submitted and to make all final determinations. Additionally, a determination by the City that a contractor is prequalified does not amount to a final determination that such contractor is responsible or responsive for purposes of bid evaluation. The City may, in accordance with applicable law reject a prequalified contractor's bid, and the City may additionally reject all bids if it determines such action is in the best interest of the public. The City may, as part of any bid for a particular project, require that contractors provide additional information regarding their qualifications to perform the project. |
