Description

Provide all equipment, materials, and labor to improve driveway pavement and drainage as follows: 1. Saw cut, remove, and replace 4,415 square feet of 2.0-inch HMAC pavement, Type D, as shown on SitePlan, Exhibit A. 2. Compaction ofin-situ soil to 95% density as perASTM D-1557. Compacted soil must be void of all vegetation. 3. Contractor to provide QualityAssurance Tester for depth and density. One density test report and one HMAC material depth test report is required for close-out, at Contractor’s sole cost and expense. Contractor to submit Type D HMAC mix order form sample. 4. Application of Prime Coat binder prior to installation of HMAC Type D pavement at an application rate of0.1 to 0.3 gal/square yard. Recommended product manufacturer by SealMaster, Terra Pave,PrimeEco, or approved equal. 5. Existing concrete header curbs to remain undisturbed. 6. Demolish and dispose of two (2) 3’x13’ concrete sidewalk culverts.Subject area to be filled with sandy loam soil, see Exhibits B and C. Install curb and gutter system at both ends, seeExhibits Band C. 7. Patch two (2) 3’x5’ portions of concrete sidewalk. All new concrete to be 3,500 psi at 28 days strength. 8. Saw cut 3’ wide openings for new drainage ponds. Grade two (2) 9’ x 150’ v-ditches at 0.05%(drainage ponds), sloped to drain as shown in Exhibits B and C, Flowlines.
