Description

All responses must be made on the forms provided. All blank spaces must be filled in, in ink or typewritten and the bid forms must be fully completed and executed (signed by an authorized individual) when submitted. The City of Alton reserves the right to reject any or all responses, or any part thereof, on any basis and without disclosure of a reason. Failure to make such a disclosure shall not result in accrual of any right, claim or cause of action by any unsuccessful respondent against the City of Alton, Illinois. The City also reserves the right to waive any irregularities or minor defects or to accept any response which in the judgment of the City may be most advantageous to the City. Although price is a major consideration, the award will not be made on price alone. The experience, references, equipment and time element of each respondent will be evaluated, as well as the City's previous experience with the respondent, if any. Award of contract will be made on the most advantageous response, as interpreted by the City Council. All other aspects of the specifications and response documents being equal, the City reserves the right to award to a respondent whose place of business is within the City of Alton provided that such respondent's price is two (2%) percent or less above the low response. Any response may be withdrawn prior to the above scheduled time for the opening of bids or authorized postponement thereof. Any response received after the time and date specified shall not be considered. The City of Alton extends to each respondent an equal economic opportunity to compete for City business and strongly encourages voluntary utilization of disadvantaged, female and/or minority-owned businesses to reflect both the industry and community ethnic composition. The City cannot accept your bid response via fax. Furnish labor, material, services, transportation, insurance and equipment necessary to expose, remove and dispose of ACM in accordance with all governmental and safety guidelines including, but not limited to requirements of the U. S. Environmental Protection Agency, Occupational Safety and Health Administration and the State of Illinois regulations. Scope of work to include the following: 1. Utilize asbestos abatement practices and procedures as prescribed and/or recommended by Federal and State regulatory agencies. The ACM will be removed from the facility by glove bag and gross removal. In addition, the basic training manual approved by the EPA and used throughout the industry for asbestos abatement entitled, "Supervision of Asbestos Abatement Projects", will be used as a guideline for conducting the work. 2. Perform daily personal samples, 30 minute personnel excursion samples, daily area air monitoring and final air clearance samples to document the amount of airborne fibers and the effectiveness of asbestos abatement engineering controls. Copies are to be kept on file with the City of Alton. 3. Transport and dispose of the ACM to an EPA approved asbestos landfill. 4. Provide documentation of abatement activities. The documentation will include copies of daily project reports, workers training, respirator fit test, landfill manifests and air monitoring results. MADISON COUNTY COMMUNITY DEVELOPMENT REQUIRES A COPY OF THE 10 DAY EPA NOTICE AND THE CANCELLED CHECK. FAILURE TO PROVIDE WILL BE GROUNDS FOR NON-PAYMENT. 5. After completion of the project, contractor shall provide the "Clearance Letter" and copies of the State of Illinois licenses showing the expiration date of the employees working on this project. Commencement of services shall be within 7 calendar days after notification. Completion of services shall be within 3-5 calendar days from commencement. Contractor shall be responsible for proper set-up, usage and removal of all necessary safety control equipment required to protect its employees and the public. The safety of the contractor's employees and the public is of prime concern to the City, and the contractor must take all necessary steps to assure proper safety during the performance of this contract. The contractor shall, at all times, observe and comply with all federal and state laws, local ordinances and regulations which affect the conduct of this work. The contractor shall be solely responsible for all claims and liabilities arising from or based upon the violation of any such laws, ordinances or regulations whether by the contractor or its employees. A Certificate of Insurance naming the City of Alton, IL and Madison County Community Development as "additional insured" and showing a minimum of General Liability (with no exclusion for asbestos) coverage of $2,000,000 aggregate. The additional insured endorsement form, along with the declaration page must be provided. The Certificate Holder shall be shown as: City of Alton, IL, Attn: Lori Bockholdt, 101 East Third Street, Alton, IL 62002 The "hold harmless" agreement included in this bid package must be signed and returned with the bid package. Obligations of the City of Alton, Illinois shall cease immediately, without penalty or further payment being required, if, in any fiscal year, the City Council of the City of Alton, County of Madison, State of Illinois funding sources or Federal funding sources fail to appropriate or otherwise make available funds for this purpose. The City of Alton, however, will use its best efforts to secure funding for the contract.
