Description | Description of Work: Location: H.W. Mountz Elementary School, 411 Tuttle Avenue, Spring Lake, New Jersey 07762 Replace eleven existing classroom unit ventilators, including the addition of DX coils, and optional external AC condensers. Bidders shall be prequalified by the New Jersey Division of Property Management and Construction in any of the trade categories listed below for the value of the work: Trade Category # Title C032 HVACR. Copies will be provided, by the above, upon providing a written request, indicating company name, address, company contact, phone number and contact e-mail address. Bid documents and all addenda will be e-mailed to the contact e-mail provided and will requiring a confirmation of receipt in writing via return e-mail. Bids must be sealed and delivered to the School Business Administrator/Board Secretary of the Spring Lake Board of Education on or before the date and time indicated below. The Board of Education does not accept electronic (e-mail) submission of bids. Site inspection is encouraged. Arrangements to visit the site are to be made through Joseph Griffin, who's contact information is listed above. This is an active school site, and no visitors are permitted without an appointment and escort. Picture identification will be required to enter the school. All bidders are required to comply with the requirements of N.J.S.A. 10:5-31 et seq., Affirmative Action Against Discrimination {N.J.A.C. 17:27-1 et seq.). An Initial Project Workforce Report will be required from the successful contractor. (Form AA-201). Statement of Ownership Requirement: Pursuant to N.J.S.A. 52:25-24.2, Bidders shall submit a statement setting forth the names and addresses of ail persons and entities that own ten percent or more of its stock or interest of any type at all levels of ownership. The bid package wili also include other documents that must be completed and returned with the bid. Failure to comply with Instructions to Bidders and to complete and submit all required forms, may be cause for disqualification and rejection of the bid. The Board of Education reserves the right to reject any or all bids pursuant to N.J.S.A. 18A:18A-18, 18A:18A2(s), (t), (x), (y), 18A:18A-4(a-c), 18A:18A-22, and to waive any informalities that may be in the best interests of the board of education. |
