Description | The sign in sheet is a separate entity from the residence and/or campus visitor log. When signing in, be sure to check the project name listed on the sign in sheet to ensure you are recording your attendance correctly. Failure to do so may cause you to be ineligible to submit a quote. There are no exceptions. Please write clearly for corresponding purposes and make sure you include an email address and phone number where you can be reached. Late arrivers at the site visit may not be allowed to sign in, participate, nor submit a quote for this project. If you are unable to attend, you will not be allowed to schedule a site visit at a later time. The Bernard Fineson DDSOO reserves the right to reject any and all quotes. In deciding whether to or not to submit a quote, please be aware the awarded vendor must adhere to the time frames outlined in the scope of work. The awarded vendor will also be required to submit certificates of Liability Insurance and Worker's Compensation Insurance as well as completed Lobbying Law Forms, and Signed Appendix A. Please be prepared to submit these in a timely fashion. prevailing wages must be paid. The NYS Department of Labor PRC number for this project is 2023007098. NYS Labor law requires that all contracted employees performing maintenance, repairs or capital improvements in state owned buildings receive the prevailing wage and supplements rate for the occupation in which they are working. If this is a public work contract covered by Article 8 of the Labor Law or a building service contract covered by Article 9 thereof, neither Contractor's employees nor the employees of its subcontractors may be required or permitted to work more than the number of hours or days stated in said statutes, except as otherwise provided in the Labor Law and as set forth in prevailing wage and supplement schedules issued by the State Labor Department. Furthermore, contractor must pay at least the prevailing wage rate and pay or provide the prevailing supplements, including the premium rates for overtime pay, as determined by the State Labor Department in accordance with the Labor Law. Contract term: 30 days of award Please note, vendors must remain for the mandatory site visit in its entirety in order to ask questions regarding the specifications of this project. Leaving the site visit early will cause you to be ineligible to submit a quote. In the event modifications are made to the project scope of work, the modifications will be sent in email to all in attendance. Any vendor questions not asked during the site visit must be submitted in writing to: Brooklyn.CMM.Bids@opwdd.ny.gov (within two business days of the site visit: July 12, 2023). Both questions and answers (if any) will be provided to all who attend the site visit via email by: July 17, 2023. The vendor shall provide all labor and materials necessary to complete the work in a professional manner according to scope of work and architectural drawings if supplied. Work is to be completed within 30 days of award, unless modified below. This is a NYS Public Works project and prevailing wages are to be paid. Site must be cleaned, and all materials secured at the end of each work day. Absolutely no scraps, especially fasteners, are to be left unsecured. Upon completion of project, before removal of equipment and scaffolding, contractor is to schedule a quality control and quality assurance inspection with Plant Superintendent.
