Description

Interior renovation to create two (2) new conference rooms, new carpet & paint, and selective repair pf the existing concrete slab on grade located in the adult services department of the existing library. The Zion Benton Public Library (hereinafter called OWNER) will receive sealed bids for the interior renovations at their existing facility. You are invited to submit a sealed bid, subject to the terms and conditions of this invitation to bid. Please read the instructions and specifications carefully. Failure to comply with these instructions may disqualify your bid. Period of Contract: The contract shall remain in effect from the approval by the Library Board of Trustees to the completion of the work, which shall not extend past September 2023. Project Site: The location of the project site referenced herein is the Zion Benton Public Library located at 2400 Gabriel Ave., Zion Benton, Il 60099 Sales Tax Exempt: No local sales, use or excise tax to be included for this project, all other applicable taxes to be paid by contractor and included in their base bid. Bid Consideration: Bid consideration will be based on a number of factors including prRequired Certifications: Each bidder is required to submit the following certifications in the form attached hereto as Rider D. Each bid must be accompanied by a Certificate of Bid certifying that the Bidder is not barred from bidding on public contracts due to a conviction for the violation of section 33E-3 (Bid Rigging) or section 33E-4 (Bid Rotating) of the Illinois Criminal Code of 1961 (720 ILCS 5/33E-3, 5/33E-4). The form for Certificate of Eligibility to Bid is included within the bid documents. No bid shall be considered responsive unless accompanied by the signed Certificate of Eligibility to Bid. ice, experience, references, compatibility of materials to existing materials in use at the Project Site, availability of maintenance or repair work, and ability to timely coordinate and complete the work assigned to that trade. Owner has the right to reject any and all bids and waive defects, technical or otherwise, that it deems to be in their own best interest. Bid Bond: A bid bond is required for all bids in the amount of 10% of the bid value issued by a surety authorized to do business in the state of Illinois or by certified check/money order. Payment and Performance Bond: A Payment and Performance bond is required for all contracts over $50,000. Bonds shall be in the amount of 100% of the contract value by a surety authorized to do business in the state of Illinois and shall meet other requirements established by law. Any bidder over $50,000 shall realize that they will need to supply a Payment and Performance bond (AIA standard form A 312 2010) and pursuant to applicable law within 10 days of being an issued a notice to proceed by the Zion Benton Public Library Board of Trustees.` Prevailing Wage: The chosen Contractor must pay its employees the applicable prevailing wage rate, in accord with the requirements of the Illinois Prevailing Wage Act ("Act") (820 ILCS 130/1 et seq.)., see attached "Rider A". All contractors and subcontractors rendering service under this contract must comply with all requirements of the Act, including but not limited to, all wage notice and record keeping duties, including the Substance Abuse Prevention Public Works Act. Insurance and Indemnification: See attached "Rider C" explaining the OWNER's insurance requirements for this project. Additionally, the contractor shall indemnify and hold harmless the OWNER and its agents to the fullest extent of the law pursuant to article "E" of the attached "Rider C" INSURANCE REQUIREMENTS. Compliance with Laws: Compliance with all applicable laws is the sole responsibility of the contractor. See attached "Rider B" COMPLIANCE WITH LAWS explaining the OWNER's compliance requirements. Required Certifications: Each bidder is required to submit the following certifications in the form attached hereto as Rider D.
