Contractor agrees to provide pest control services as required for Region 03, for locations including but not limited to: 1. Counties within in HHSC Region 03: Dallas, Tarrant, Fannin, Johnson, Navarro, Denton, Wise, Hunt, Kaufman, Collin, Palo Pinto, Rockwall, Grayson, Earth, Ellis, Parker, Cooke, and Hood. 2. Cities within in HHSC Region 03: Arlington, Bonham, Carrollton, Cleburne, Dallas, Decatur, Denton, Duncanville, Ennis, Fort Worth, Grand Prairie, Granbury, Greenville, Hurst, Irving, Kaufman, Lake Worth, Lewisville, McKinney, Mesquite, Mineral Wells, Plano, Richardson, Sherman, Terrell, Watauga, Waxahachie, and Weatherford. 3 The following locations shall be treated monthly when requested by the agency: A. 1205 E Sam Rayburn, Bonham, Texas 75418 B. 2700 Ben Ave., Fort Worth, Texas 76103 4 The following location shall be treated twice a month when requested by the agency: 5. 6500 Northwest Dr., Suite 300, Mesquite, Texas 75150
