Owner: The Jeffersonville Housing Authority Jeffersonville, Indiana The Pre-bid Conference is for the Purpose of Permitting Bidders and Their Subcontractors to Thoroughly Inspect the Areas of the Proposed Work and to Request Clarifications or Interpretations of Contractor's Questions. Any Responses to Material Concerns Shall Be as Issued in an Addendum to the Contract Documents. 1.3 Bids Will Be Received From Contractors for Work as Shown on the Drawings and Hereinafter Specified and as Set Forth on the Contractor's Bid Form. All Contractors Considering Submitting a Bid Should Verify With the Reprographic Printing Company Listed Above, the Total Number of Addenda Issued on a Project the Day Before the Proposal Due Date. All Other Data Required by Bid Form 96, Part Ii and Part Iii Must Be Submitted as Required by Bid Form No. 96 Without Regard to the Total Bid Amount. 1.11 Each Individual Bid Package Proposal Shall Be Accompanied by a Certified Check or Bank Draft, Payable to the Jeffersonville Housing Authority, Jeffersonville Indiana. Government Bonds, or a Satisfactory Bid Bond Executed by the Bidder of an Acceptable Surety In an Amount Equal to Five Percent (5%) of the Base Bid Plus All Add Alternates. 1.12 Sealed Envelope Shall List the Name and Address of the Proposal Bidder on the Exterior of the Envelope. 1.13 the Successful Bidder Shall Be Required to Provide a 100% Performance Bond and Labor and Material Payment Bond Before Starting Any Work or Placing Materials on the Job. The Bonds Shall Be Received Within Ten (10) Days of Receipt of Notice to Proceed. 1.14 All Prices Bid, Are to Be Free of All Indiana State Sales Tax, and Upon Request of Supplier, a Tax Exemption Certificate Will Be Executed by the Owner to the Successful Bidder to Cover All Items Subject to This Tax. 1.15 Attention is Called to the Provisions for Equal Employment Opportunity, And Payment of Not Less Than the Minimum Davis Bacon Wage Rates And Salaries Must Be Paid on This Project, as Set Forth in the Project Manual. 1.16 Attention is Called to the Fact That the Contractor Must Take Affirmative Action to Insure That Applicants Are Employed, and That Employees Are Treated During Employment, Without Regard to Their Race, Color, Religion, Sex or National Origin. 1.17 Contractors Are Required to Comply With the Provisions of Section 3 as Set Forth on 24 Part Cfr B5 as Follows: A. A Good Faith Effort to Utilize Section 3 Area Residents as Trainees And Employees. B. A Good Faith Effort to Award Contracts for Work in Connection With the Project on Section 3 Business Concerns. 1.18 Attention is Called to a Request That the Contractor Shall Make a Best Efforts Goal to Achieve a State Goal of 10% Minority and/or Women Owned Business Enterprise Participation in This Project. 1.19 Bidders Are Required to Submit the Following Documentation, in Triplicate, With Each Individual Bid Package Submittal: 1. Contractor's Bid for Public Works, State Form 96, as Prescribed by The State Board of Accounts of Indiana, Complete With Parts I - Iii. These Completed Forms Are a Required and Mandatory Document for This Project, Regardless of the Actual Bid Amount Total. 2. Hud Form 5369a, Representatives Certification and Other Statement of Bidders. 3. Hud Form 2530, Previous Participation Certificate. 4. Hud Form 5370, Contractor's Insurance Certificates With Coverage And Limits as Stated in the General Conditions. 5. Stated Acknowledgement of All Addenda Issued on the Project. (Space Provided on the Bid Forms) 6. Contractor's Financial Statement (Mandatory, as Required in Part Ii Section Iii of State Form 96) 7. Contractor's Bid Guarantee: Bid Bond, Certified Check, or Bank Draft. 1.20 Bids May Be Held by the Jeffersonville Housing Authority for a Period Not to Exceed Forty Five (45) Calendar Days From the Date of the Opening of Bids for the Purpose of Reviewing the Bids and Investigating The Qualifications of Bidders Prior to Awarding the Contract. Bidders Shall State in Hud Form 5369a, Article 6(D) if a Longer Acceptance Period is Offered. 1.21 the Jeffersonville Housing Authority Reserves the Right to Reject Any or All Bids or to Waive Any Informalities in the Proposal Solicitation Process, to the Extent Allowable by Law.
