Description

Request for Proposal to Construct Replace Roof, Various Locations, Main Hospital Project No. 667-18-104 Overton Brooks VA Medical Center, Shreveport, LA * REVISED 08/10/2022 SCOPE: Overton Brooks VAMC has a requirement to replace several roofs at various locations (approximately 30,000 SF) on the main hospital, building 1 Shreveport, LA. Roof sections include, but are not limited to, (A) Second Floor Clinical Addition, (B) Lobby, (C) Second Floor South, (D) 8-West, (E) 9-West, (F) Second Floor Canteen, (G) 12th Floor and (H) 10-North old Cooling tower area (Screen Wall). All asbestos containing material shall be abated. The existing roof systems shall be as built showing all roof penetrations, roof drainage piping, heating ventilation and air conditioning (HVAC) equipment, ductwork and appurtenances of any type. Any items or systems determined to be abandoned or no longer required shall be identified to be removed and capped or terminated. Access and continued function/operation of adjacent occupied spaces (above, below and beside) shall be considered within the construction phasing plan to minimize the overall impact and disruptions generated by the construction activities. The C&A requirements do not apply, and that a Security Accreditation Package is not required. PERIOD OF PERFORMANCE: The delivery date for this contract shall be 200 days from Notice to Proceed. CONTRACTOR SITE VISIT AND FIELD VERIFICATION PRIOR TO BIDDING: Contractor shall be responsible for attending the Pre-Bid Site Visit prior to bidding to verify all existing conditions and take any necessary measurements required to adequately bid the project. If the Contractor chooses not to attend the Pre-Bid Site Visit, the Contractor may submit their bid at their own risk. INFECTION CONTROL DURING CONSTRUCTION: Contractor shall follow and comply with the established Infection Control Risk Assessment (ICRA) specific for the project. Contractor is responsible for dust control during demolition and during hauling operations. DEMOLITION: Contractor shall protect all adjacent areas of the hospital, especially windows and doors during all operations. Repairing any damage shall be the sole responsibility of the Contractor. If any damage is noticed prior to starting work which may be misconstrued as damage caused by the contractor, immediately notify the COR, and the damage shall be documented as existing conditions. PHASING, NOISE, SMELLS, AND AFTER-HOURS WORK: Work resulting in extensive noise or odor, shall be performed after hours or on weekends. Any required utility outages shall be scheduled at the convenience of the hospital, and potentially require after hours work or on weekends. DELIVERIES: Contractor shall coordinate material and equipment deliveries, storage, and handling requirements with COR. FINAL CLEAN, PRE-FINAL & FINAL INSPECTIONS: Once all work has been completed, the Contractor shall adequately clean the project site and request a Pre-Final Inspection. Upon completion of the pre-Final punch list items, the Contractor shall request a Final Inspection. Each of these inspections will be scheduled at the convenience of the hospital. SUBMITTALS: Provide: Email an electronic copy to the COR and each copy shall have a completed VA transmittal sheet attached. Contractor shall provide submittal as detailed in the specifications and samples of all finishes showing the full range of available colors and options to be selected by the VA-ID and COR. *The value for this project is based on a financial range. The value is listed as the highest possible cost from the range provided by a stakeholder or official project document.
