Description | 1. Contractor to provide all labor, materials, equipment and supervision required for work shown Dwg-01 through Dwg-03 and within this scope of work. See Gen Notes on Dwg03. 2. Contractor is responsible for providing a safe worksite and any safety items necessary such as traffic control, flagger, barricades, safety flagging, signs, etc. Traffic control shall be performed by contractor per MUTCD (current edition) requirements for shoulder work and lane closures. Flaggers shall be required when lane closures are necessary. Contractor shall inform City Engineering Department a minimum of 24 hours prior to any proposed lane closures. 3. Contractor to provide a port-o-let for the duration of the project. Cost to be included in mobilization. 4. Contractor is responsible for underground locate request. Any utility conflict shall be brought to City's attention immediately. 5. Contractor is responsible for layout work. 6. Contractor to remove existing topsoil and organic material and set aside for re-use. Excavate to required grade to achieve ditch slope and place non-woven filter cloth and 4” thick rock base ditch bottom and compact. 7. Excavate for side slopes and place non-woven filter cloth and sand clay fill approximately 4” thick and shape to sidewall profile and compact. 8. Place WWM mats using plastic or wire chairs. Allow for 12” lap to sidewalls. 9. Place concrete for ditch bottom and install expansion material at construction joints as shown on drawings. 10. Place WWM along sidewall slopes using plastic chairs and tie to WWM lap from ditch bottom. 11. Place concrete to construct sidewalls. Place expansion material as shown on drawings. 12. Place filter cloth and rip-rap as shown on Dwg-01 13. Re-Use and grade existing topsoil as required 14. Fine grade and place sod around new concrete channel. 15. Contractor responsible for removal of all debris from worksite upon completion of the project. 16. Contractor is responsible for watering of sod for a period of 30 days. 17. It is the contractor’s responsibility to contact the appropriate City Department(s) to inquire about permits and/or licensing requirements and fees for this project. |
