Capitol City Family Health Center, Incorporated d/b/a CareSouth (“CareSouth”) is a 501(c)(3) non-profit Federally Qualified Health Center (“FQHC”) operating multiple sites in five (5) cities across Louisiana: Baton Rouge, Donaldsonville, Plaquemine, Shreveport, and Zachary. CareSouth provides medical and dental services to the community. CareSouth is seeking proposals from qualified companies to provide HVAC Maintenance Service for a period of at least one year beginning February 1, 2023, including but not limited to monthly filter change, belt replacement, and cleaning of condensers & evaporator coils for the following locations: • 3140 Florida Blvd., Baton Rouge, LA (“Baton Rouge Clinic”) • 3111 Florida Blvd., Baton Rouge, LA (“Corporate Office”) • 3324 Florida Blvd., Baton Rouge, LA (“PEDS”) • 904 Catalpa Street, Donaldsonville, LA • 59340 River West Drive, Plaquemine, LA • 4852 Hwy 19, Zachary, LA CareSouth is accepting sealed proposals for Heating, Ventilation and Air Conditioning (HVAC) services. Sealed proposals clearly labeled Heating, Ventilation and Air Conditioning (HVAC) must be postmarked no later than January 13, 2023 at CareSouth, 3111 Florida St., Baton Rouge, LA 70806 or may be dropped off in-person no later than January 13, 2023.
