Description

City Hall Underground Storage Tank (UST) Removal and Replacement - City Hall (1420 Miner St.); If any prospective bidder has questions about this Request for Proposals, contact Tom Bueser at 847-391-5470 between the hours of 7:30 A.M. and 3:30 P.M. or via Email at: tbueser@desplaines.org All contract proposals for the Work shall be made only on the Contract form attached to this Request for Proposals and shall be complete with a price for each and every item named in the Schedule of Prices section of the Contract form. All contract proposals must be dated on Page 5 of the Contract form and must be signed by an authorized official. All contract proposals shall be accompanied by a cashier's or certified check, or bid bond in form and from a surety satisfactory to Owner, in the amount of 5% of the total cost submitted. Proposals that contain omissions, erasures, alterations, or additions not called for, conditional or alternate bids unless called for, or that contain irregularities of any kind may be rejected. Owner reserves the right to reject any and all proposals, reserves the right to reject the low price proposal, and reserves such other rights as are set forth in the Instructions to Bidders. Proposal: The work shall include the removal of removal of the existing 1,050-gallon and 4,000-gallon underground fuel storage tanks and replacement with one 5,000-gallon double wall ZCL-Xerxes fiberglass underground fuel storage tank for three generators, associated double wall piping, manholes, spill containment, overfill valve, tank sump, and new fuel leak detection monitoring system with training. Work will be conducted at 1420 Miner St., Des Plaines, IL 60016. - Remove and provide proper disposal for the existing 1,050-gallon and 4,000-gallon underground fuel tanks. - Provide environmental site assessment, environmental early action 20-45-day reports, and all soil testing - Pump out and dispose of remaining product - Disconnect pumps - De-fume tanks to proper LEL levels - Excavate and remove tank with all related piping - Place tanks on visquene - Cut off ends of tanks - Properly clean interior of underground storage tanks in accordance with Office of the Illinois State Fire Marshal requirements - Haul away underground storage tank and all related piping - Provide certificate of tanks destruction to owner - Furnish and install one new 5,000-gallon capacity double wall fiberglass ZLC-Xerxes underground storage tank complete with all associated double wall piping, manholes, spill containment, overfill valve, and tank sumps. - Furnish and install one fuel leak detection monitoring system Petro-Vend or approved equal and associated training; not less than 6 hours. - The Contractor shall test the completed tank system including the alarm and leak monitoring systems in accordance with Federal and State standards and in accordance with the manufacturer's Installation Manual and Operating Guidelines in effect at the time of installation. - Place all backfill back into excavation - Fill in tank voids with CA6 stone compacted to grade level - Price includes on site environmental geologists to document procedure, provide pictures of work in progress, and underground storage tank removal report with samples for testing if necessary- - The Contractor shall remove and provide proper disposal of any existing reinforced concrete removed for the placement of the new underground fuel storage tank. All existing piping, electrical sensors and wiring shall be located and protected. - For bidding purposes area of excavation and replacement is approximately 640 square feet. Payment will be for actual square feet replaced; additional replacement cost is included in schedule of prices. - All work shall be in accordance with the latest edition of IDOT, Standard Specifications for Roads and Bridge Construction to match existing conditions. Granite ribbon shall be saved onsite for re-use. - Four test cylinders shall be made to provide sample test results for 7 day and 28 day compressive strength. The Contractor shall obtain approval of the testing firm from the Owner prior to sampling. The cost of the testing shall be included in the Contractor's bid. - All material and workmanship shall be warranted against defects in material and labor for 1 year after acceptance of the project by the Owner. The tank and apparatus shall have the manufacturer's standard warranty for materials and workmanship. All manufacturers' - warranties shall be provided to the Owner at job completion.
