ADD ALTERNATE NO. 1 - VALEMONT Approximately 6,200 SY of 3" (+/-) Roadway Milling and Profiling, 315 S.Y. of Soft Spot Repair, 840 Tons of Superpave 19 mm Binder Course, 6,200 S.Y. of 1.5" Superpave 9.5mm Fine Graded Wearing Course, 4,200 LF of Bituminous Wedge Curb, 1 Storm Inlet Replacement, 1 EA Manhole/Inlet Adjustments, Traffic Control, Resident Notification and Restoration. ADD ALTERNATE NO. 5 - GREENVIEW Approximately 3,700 SY of 3" (+/-) Roadway Milling and Profiling, 200 S.Y. of Soft Spot Repair, 500 Tons of Superpave 19 mm Binder Course, 3,700 S.Y. of 1.5" Superpave 9.5mm Fine Graded Wearing Course, 2,100 LF of Bituminous Wedge Curb, 40 LF of Underdrain, 2 EA Connection of Roof Drains to Underdrain, Traffic Control, Resident Notification and Restoration. All bidders must be PennDOT pre-qualified as a prime contractor and must be assigned and eligible to bid the work classification codes of E, F, and F1. All bids submitted through Pennbid must include the contractor's valid pre-qualification certification or the bid shall be rejected. Pennsylvania Prevailing Wages are required to be paid on this project. Proposals to receive consideration must be accompanied by a Certified Check or Bidder's Bond from a Surety Company authorized to do business in Pennsylvania, made to the order of the Municipality of Penn Hills in an amount equal to ten percent (10%) of the total amount of the Proposal as a guarantee that, if the Proposal is accepted, the successful Bidder will enter into an Agreement within 15 days after Notice of the Award of the Contract. The Proposals must be made to the Municipality of Penn Hills, Allegheny County, Pennsylvania, and shall remain firm for a period of ninety (90) days. No Bidder may withdraw his Proposal during the ninety (90) day period without forfeiting his Bid guarantee. Performance, Maintenance, and Labor and Material Payment Bonds, along with Public Liability and Property Damage Certificates of Insurance in the amounts specified, as well as Certificates of Workman's Compensation must be filed with the executed Agreement upon acceptance of the Proposal from the successful Bidder. The Owner reserves the right to reject any or all Proposals, or any part thereof, for any reason, and also reserves the right to waive any informality therein.
