Project #: 673-22-109 IAW FAR 36.204 & VAAR 836.204 the VA's magnitude of this construction project is between $1,000,000.00 and $2,000,000.00 NAICS: 236210 - Industrial Building Construction Product Service Code (PSC): Z1DA - Maintenance of Hospitals & Infirmaries This solicitation will be evaluated on a trade-off basis. As stated elsewhere in this solicitation, this procurement is a two (2) phase design/build IAW FAR 36.3. One solicitation will be issued covering both phases. In Phase I, proposals will be evaluated to determine which offerors will submit Phase II proposals. Block 13 A immediately below only refers to Phase One proposals. This requirement is set-aside for VIP certified, Service-Disabled Veteran Owned Small Businesses (SDVOSB). Vendor verification for SDVOSB's can be found at https://www.vetbiz.va.gov. ANY PERSON OR ENTITY SUSPECTED OF MISREPRESENTATION FOR THE PURPOSE OF SECURING A GOVERNMENT CONTRACT MAY BE CRIMINALLY INVESTIGATED AND PROSECUTED FOR FRAUD AGAINST THE UNITED STATES GOVERNMENT. James A. Haley VAMC is a smoke free facility. Contractors, in order to enter the campus, you must have a picture ID. All contractors are required to wear a face covering (per guidelines) while on campus. All contractors will be screened at the entrance of the VA campus. A series of questions will be asked and temperatures taken. While working inside or outside the VAMC, the contractor will be responsible to wear a face covering at all times. Personal Protective Equipment (PPE) is not provided. Refer to solicitation instructions for Phase One cutoff date for submission and Request for Information (RFIs), or questions, if any. No site visit will be conducted during Phase One of this solicitation. All offerors must comply with all solicitation terms and conditions (T&C's). Please refer to the instruction contained in RFP 36C24823R0068 for submission requirements. Davis Bacon Wage Rate FL20220208 dated Sep 09, 2022 applies. Furnish all necessary professional services, equipment, labor materials, supervision, tools, testing, and specialty services to perform a design-build for the NRM Project #673- 22-109, "Install Fan Array AHU OR 2-7 and Replace Exhaust Fans," at the James A. Haley VA Hospital, 13000 Bruce B. Downs Boulevard, Tampa, FL 33612. The work includes, but is not limited to site investigation, design and preparation of drawings, phase planning, specifications, construction, and installation as designed in the hospital areas identified in the project description in this SOW.
