Description | Mailed bids shall sent to the Purchasing Director with a Label indicating "BID ENCLOSED -"Rehabilitation of Case Avenue Bridge over Mad River" to the following address: Mark Douglass Purchasing Agent; Town of Winchester; 338 Main Street; Winsted, Connecticut 06098. Bids shall be submitted in a sealed envelope inside of the Mailing Envelope. Bids must arrive before the time and date above. NO EXCEPTIONS. FEDERAL PROJECT F.A.P. # 6162(015) CN Town of Winchester State Project No. 0162-0157 Town Project No. N/A Rehabilitation of Bridge No. 05127 Case Avenue over Mad River Contract Goal: 8% Disadvantaged Business Enterprises (DBEs) The Town of Winchester (Municipality) hereby notifies all bidders that it will affirmatively ensure that in any contract entered into pursuant to this advertisement, Disadvantaged Business Enterprises will be afforded full opportunity to submit bids in response to this invitation and that they will not be discriminated against on the grounds of race, color, national origin, sex, sexual orientation, mental retardation or physical disability, including but not limited to blindness, in consideration for an award. "Bid Proposal Form", "Schedule of Prices", "Bid Bond", "Non-Collusion Affidavit" and "Bidder's Prequalification" approval letter signed by the CTDOT Contracts Manager must be completed and returned with the submitted bid. Failure to properly execute and include any one of these five documents in the bid submission will result in the bid not being read and the subsequent rejection of the bid. Bidders are responsible for all printing costs and shipping costs, as applicable. Bid Documents are not available for purchase at the Town. Form 818 Standard Specifications may be obtained via the Connecticut Department of Transportation ELECTRONICALLY All Proposals must be on the form furnished by the Municipality and must be requested for the above-named project. NOTE: Prospective bidders must have a current sworn Statement (CON-16) on file with the Connecticut Department of Transportation and be prequalified to perform Group No. 8 (Minor Bridges) work. The Bidder's Prequalification approval letter signed by the CTDOT Contracts Manager shall be included as part of the bid package submitted to the Municipality. NOTE: Any Contractor or Subcontractor engaged in surface preparation and/or coating application must be certified by the Steel Structures Painting Council. NOTE: A Surety Company Bond, on the form furnished by the Town of Winchester, for at least 30 percent of the amount of the bid must accompany each proposal. A certified check will not be accepted. The Municipality reserves the right to reject any and all bids. NOTE: The two (2) lowest bidders, as determined by the Town of Winchester immediately after the bid opening, shall submit the Pre-award DBE Commitment Approval Request form(s) to the Town of WinchesterNO LATER THAN FIVE (5) calendar days after the bid opening. This is a requirement of Title 49, Code of Federal Regulations (CFR) Part 26, Participation of DBEs.
