Description

New Safety and Security Window Film at the Countryside and Hillside Schools. Each successful bidder shall be required to furnish a performance payment and completion bond from a surety company approved by the Owner, the form and content of which shall comply substantially with the form prescribed by New Jersey statutes for public agencies, and as amended and amplified herein. Each bidder shall comply with the requirements of Title 18A: - 18A-27 et seq. Public School Law, which pertain to the prequalification of bidders. The notice of classification and the amount of uncompleted contracts form shall be enclosed in a separate sealed envelope, along with the bid proposal. Bids must be made upon the Proposal Form provided in the manner designated. The name of the bidder, his address and category of work bid upon must appear on the outside of the sealed envelope and must be accompanied by a proposal guarantee in the form of (1) a certified check or cashier's check made payable to the Mt. Laurel Board of Education in the amount of ten percent (10%) of the proposal, but in no case in excess of Twenty Thousand Dollars ($20,000), or (2) a bid bond in the amount of ten percent (10%) of the proposal. With the bid there shall be furnished a "Consent of Surety" or "Bid Letter" from the bonding company stating that they will provide payment, performance and completion bond for 100% of the contract price if contract is awarded. Pursuant to NJSA 52:34-15, Bidder shall submit with his bid the Non-Collusion Affidavit on the form as included in the specifications. Bidders are required to comply with all requirements of P.L. 1975, c.127 (N.J.A.C. 17-27). Submit Affirmative Action Acknowledgement on the form as included in the specifications. The New Jersey Prevailing Wage Act applies to this project, and the stipulated minimum wages that apply in this locality shall be made a part of the contract. The Contractor, where applicable, shall comply with Regulations promulgated by the Treasurer of the State of New Jersey, pursuant to P.L. 1975, c.127, as amended and supplemented. The contractor and the subcontractors named in this proposal must be pre-qualified with the "School Development Authority". The Mount Laurel Board of Education reserves the right to consider the bids for sixty (60) days after receipt thereof, and further reserves the right to reject any and all bids and to waive any informalities in any bid or bids to accept such bid or bids and to make such awards as may be in the best interest of the Owner. NOTE: BE IT ENACTED by the Senate and General Assembly of the State of New Jersey: 1.No corporation or partnership shall be awarded any contract nor shall any agreement be entered into for the performance of any work or the furnishing of any materials or supplies, the cost of which is to be paid with or out of any public funds, by the State, or any county, municipality or school district or any subsidiary or agency of the State, or of any county, municipality or school district or by any authority, board or commission which exercises governmental functions unless prior to the receipt of the bid or accompanying the bid of said corporation or said partnership, there is submitted a statement setting forth the names and addresses of all stockholders in the corporation or partnership who own 10% or greater interest therein, as the case may be. If one or more such stockholders or partners is itself a corporation or partnership, the stockholders holding 10% or greater interest in that partnership, as the case may be, shall also be listed. The disclosure shall be continued until names and addresses of every noncorporate stockholder and individual partner exceeding the 10% criteria established in this Act has been listed. 2. This Act shall take effect immediately. Drawings, Specifications and Form of Proposal for the proposed work are on file in the office of Wayne A.
