Tuba City Outpatient Treatment Center Motorized Gate Project

Scope of Work: 1. Install Two (2) Ten-foot wrought iron cantilever gates, replacing an existing 20’ cantilever drive in gate. The gates are to be 10’ high wrought iron gates. 2. Install Two motorized gate openers/closer to the new 10’ cantilever drive in gates. 3. Install a concrete pad for the gates to roll on. 4. Install two (2) pedestal type stands for a keypad (code) gate opener on the driver side of each side of the fence/ gate –accessible by a driver from their vehicle. Intercom and remote opener from the inside of the building to be included. 5. Install necessary electrical source for motorized mechanism for gates to the main breaker box inside the building for a dedicated electrical connection. 6. Install a Four-foot walk in gate directly west of the drive-in gates complete with gate posts and gate handle locking mechanisms. 7. All material is to be commercial grade. 8. Warranty information for materials and workmanship to be provided. 9. Maintenance information will be provided to staff. 10. Contractor is responsible for field verification of measurements and conditions
