Description | The Division of Behavioral and Mental Health Services (DBMHS) is requesting inspection services for existing fire alarm systems at the Fort Defiance Outpatient Treatment Center. It is the intent of DBMHS to bring all fire alarm systems up to date and in compliance with building and life safety codes. The vendor must be licensed as a commercial fire safety inspection company. The “Fire Alarm System” as-built drawings are available. Locations: • Fort Defiance Outpatient Treatment Center, Fort Defiance, AZ Building Property #6905 approximately 9,900 sq. ft. This project will entail the following: 1. Inspection and testing of an EST iO500 fire alarm control panel and components (dual dialer line; two 12 volt 7 amp batteries; an addressable manual pull station; 58 addressable photo type smoke detectors; 6 addressable relay modules; 6 ancillary relays; 6 selectable candela strobe only; 17 selectable candela ceiling mount horn/strobes and 6 fire smoke dampers.) 2. Connect to a Fire Alarm Monitoring services company. 3. Recommendation of special fire suppression system in the data room for sensitive computer and other electronic equipment. 4. Inspect and certify five (5) fire extinguishers to include recharge/rebuild if needed per NFPA 10. 5. Inspect and certify ten (10) exit lighting and emergency exit lighting and signage. 6. Inspect and certify ten (10) exit doors with panic release hardware (in good working order). 7. Location, ownership and testing of nearest public fire hydrant. 8. Provide project manager with detailed Inspection reports and language that can be utilized for the development of a scope of work for each site for repairs, replacement or additions. This report is to include costs. 9. Upon completion of inspection services, minor repairs equal to or less than One Thousand Dollars ($1,000.00) allowance per year. (Include in your cost proposal). 10. Annual inspections of all mentioned systems for five (5) years to ensure proper operations of all systems and components. 11. Provide all materials (operating/maintenance manuals) and workmanship warranty information to site. 12. Provide an overview, training and maintenance information to site staff and supervisor. 13. Contractor is responsible for field verification of systems at all sites. 14. Cost details breakdowns are to include labor, equipment, materials, minor repairs allowance, travel expenses and the Navajo Nation Tax of 6%. Please be cognizant that the grand total is the contract total. 5 15. Exclusions: Based on your expertise- please list any and all fire related life safety code requirements not otherwise in this scope of work. (Do not list costs with main proposal). |
