Description

A permit was applied for on January 12, 2023. Mad Dog Construction, the Construction Manager is seeking Sealed Subcontractor Bids from qualified firms for the project and scopes referenced below. PROJECT NO: MSFM 02103230 PROJECT NAME & LOCATION: FDLE Main, Envelope Repairs Robert R. Dempsey Building Tallahassee, Florida SCOPES: 7-A Exterior Waterproofing & Restoration 7-B Roofing MANDATORY PREQUALIFICATION: This prequalification procedure has been established in compliance with Rule 60D-5.004, FAC. All Bidders must pre-qualify regardless of any previous prequalification by three (3) working days prior to time of bid opening. Prequalification forms and bid documents may be obtained by sending an email request to Meredeth at meredeth@morethanbuildings.com . PUBLIC ENTITY CRIME INFORMATION STATEMENT: Under section 287.017 Florida Statutes, a person or affiliate who has been placed on the convicted vendor list following a conviction for a public entity crime may not submit a bid on a contract to provide any goods or services and/or construction or repair of a public building or public work and may not be awarded or perform work as a contractor, supplier, subcontractor, or consultant under a contract with any public entity for a period of 36 months from the date of being placed on the convicted vendor list. DISCRIMINATION: DENIAL OR REVOCATION FOR THE RIGHT TO TRANSACT BUSINESS WITH PUBLIC ENTITIES: Under section 287.134(2) Florida Statutes, entities or affiliates who have been placed on the State of Florida's discriminatory vendor list may not submit a bid or proposal on this contract. COOPERATION WITH THE INSPECTOR GENERAL: Pursuant to section 20.055(5), Florida Statutes, the bidder who is awarded the contract and its subcontractors understand and will comply with their duty to cooperate with the inspector general in any investigation, audit, inspection, review, or hearing. PRE-BID MEETING: TBD all bidders of record will be notified when the Pre-Bid Meeting is scheduled. All submitted quotations or bid proposals are made to Mad Dog Construction, the owner is not a party. The award of each subcontract will be based solely on the best interest of the project as determined by Mad Dog Construction.
