Description | Job NO. 21815 Street Repair Program - Phase 4 within the City of Middleburg Heights The work generally includes pavement planning/pavement profiling; single chip seal; asphalt overlay; sidewalk and curb ramp removal and replacements; curb removal and replacements; and pavement markings. Each bid must be made upon proposal forms provided in each set of specifications and contain the full name of every person, firm or corporation interested therein and be accompanied by a bid guaranty in the form of either a certified or cashier's check drawn on a solvent bank in the State of Ohio, together with a consent of surety, in the amount of (10%) of the total bid, made payable to the City of Middleburg Heights, Ohio or a bond in an amount equal to one hundred percent (100%) of the total bid drawn in favor of the City of Middleburg Heights, all as prescribed in O.R.C. 153.54, and as a guarantee that, if the bid is accepted, a contract will be entered into with the City, and in default, thereof, said check or bond will be forfeited to the City of Middleburg Heights as liquidated damages. Bidders must use the printed forms of proposal available as aforesaid, as none other will be accepted. Contractors shall be required to pay prevailing wages on this contract in accordance with Chapter 4115 of the Ohio Revised Code. The Contractor shall make the necessary adjustment in the prevailing wage rate and pay any wage increase during the term of the contract. Information on wages may be obtained through the Ohio Department of Commerce, Labor and Workers Safety, Wage and Hour. Each bidder must ensure that all employees and applications for employment are not discriminated against because of their race, creed, color, sex or national origin. The rights are reserved by the City of Middleburg Heights to reject any or all bids, or parts of any bid. |
