Description | As per the Owner the project has been cancelled. Editorial Content Team is actively seeking updated information. This contract is subject to the electronic payment process by the Department of Treasury. All invoices complying to the requirements at 52.212-4(g) must be submitted through www.ipp.gov. For additional information, refer to the incorporated clause at 352.232-71 Electronic Submission of Payment Requests (FEB 2022). Period of Performance: 09/01/2022 to 12/31/2022 Contractor shall replace water valve concrete collars, damaged sidewalk, damaged concrete sidewalk culvert, and street handicap ramp in accordance with the attached Statement of Work for the Yucca Meadow Housing compound of the Four Corners Regional Health Center in Red Mesa, Arizona. Requisition No: IHS1442152 The offeror shall e-mail questions concerning sensitive technology to the Department of State at CISADA106@state.gov. INTRODUaION: Contractor 5hall provide all labor, materials, equipment, transportation, expertise and service required to complete all work as indicated in service request. Contractor shall clean up and remove all demo debris caused by contractor operation. Approved Funded FEPPS Work Plan Project 4 NA18SR217Q# SERVICE TO BE PROVIDE: Replace water valve concrete collars, replace damage sidewalk, replace damage concrete sidewalk culvert and Street handicap ramp with damage asphalt. 1. Water Valve Concrete Collars: o Remove existing damage concrete collars at 10 location in asphalt streets. Cut out asphalt 3'x3' and over-excavate around existing valve sleeve to where 5leeves can be adjusted. Adjust valve sleeve and cover to finish asphalt. 2. Installation Concrete Collars: o Replace concrete 3'x3'xS" mm. square concrete collars. Reinforcement, concrete and subbase shall be engineered design. o Rock Point Dr.: Sea. o Rock Point & Sweetwater St.: 1 ea. o Carrizo Dr. & Sweetwater St.: 3 ea. o Carrizo Dr.: lea. 3. Backfill and compaction: o Backfill shall be suitable material and free of debris, roots, scrap material, and organic matters. Compaction shall be use mechanical tamper for compaction. 4. Replacement concrete sidewalk with curb, handicap, and sidewalk culvert: o Remove 2ea damage sidewalk with curb 7'-6"x 6' and lO'-6"x6'. lea handicap ramp with curb and damage asphalt 18'x9'-6" plus 4'x4'x4" sidewalk. Zea sidewalk culvert with curb l4'x6'. o White Mesa Ct. Cal-De-Sac. : 2ea. Sidewalk Repair o Carrizo Dr. : lea. Storm Drainage o Rock Point Dr. : 1 ea. Storm Drainage o Rock Point Dr. :1 ea. Handicap Ramp 5. Installation concrete: o New sidewalk, curb, ramp and sidewalk culvert shall be in place as original plans and detail. Preparation of subgrade shall be compacted, concrete shall be placed continuously and shall be thoroughly tamped with floated to a smooth and even surface and given light broom finish. The finished surface shall be tool jointed into squares with sides equal to existing sidewalk or walkways width. Concrete shall not be damage in removing forms. Attachment A - Statement of Work Finish tool on all expose edges. Concrete shall be engineered design for sidewalk, ramp and sidewalk culvert. PERIOD OF PERFORMANCE: March01, 2022 to November 30, 2022 LOCATION OF WORK: Yucca Meadows Housing Four Corners Regional Health Center HWY 160 & Navajo Route 35, Red Mesa Az, 86514 WHEN WORK IS TO BE PERFORMED: The service shall be conducted during normal business hours. Normal business hours are: Monday to Friday 8:00am to 4:30pm. Except federal holiday. COM MU N ICATI ON: Contractor shall coordinate with point of contact for any requesting information. Contractor shall coordinate schedule and two weeks' notice prior of arrival.
