Description

All successful bidders will be required to furnish a Performance Bond and a Labor and Material Bond pursuant to Sections 136 and 137 of the State Finance Law, each for 100% of the amount of the Contract estimated to be between $500,000 and $1,000,000 for H, and between $100,000 and $250,000 for E. The purpose of this project was to conduct a pre-renovation hazardous materials survey for asbestos, lead-based paint (LBP), and polychlorinated biphenyls (PCB) at 1631 Hollow Road, Clinton Corners, NY (The Site). The areas inspected were limited to the interior and exterior of the building only in areas expected to be impacted by the proposed HVAC Upgrades. No roof samples were taken as it was reported that the roof is being replaced under a separate contract. Ambient Environmental, Inc. (Ambient) provided the following services for Friedman Fisher Associates, P.C. (Client) in accordance with Ambient proposal number 2021-02-0072. On August 3, 2022, Ambient returned to the site to collect a sufficient amount of the red caulk on the boiler flue, in order to determine if the material contained PCBs or not. Any changes to this report of a result of this sampling are reflected in red throughout. Conduct a representative Hazardous Materials Survey in the identified building, which includes: o Survey the site building. o Identify accessible suspect asbestos-containing materials (ACMs) that were not previously tested using limited destructive means. o Quantify ACMs, including material condition and location. o Collect and analyze bulk samples of suspect friable and non-friable materials to eliminate suspect materials as asbestos containing. o Conduct a limited lead-based paint inspection of the building with a Heuresis Corporation Pb200i Lead Paint Analyzer. o Collect and analyze bulk samples of potential PCB containing window/door caulk. 2.0 EXECUTIVE SUMMARY The inspection was conducted by NYS licensed and AHERA trained asbestos inspectors and trained lead inspectors. The inspection involved visual examinations and sampling of suspect materials that may be impacted by planned renovation projects. Inspection results revealed the following findings: o The renovation area does contain asbestos containing materials o The renovation area does contain lead-based paint o The renovation area does not contain PCBs in caulking o In the attic there is an asbestos incidental disturbance of pipe insulation and mudded fittings. This disturbance will require a contamination assessment and the potential development and submission of a NYS DOL Site Specific Variance to allow for the clean-up and abatement of this material by a NYS licensed and certified asbestos abatement contractor. This area should be cordoned off and not occupied by any uncertified asbestos personnel until the proper cleanup has been completed. . ELATED CONTRACTS A. The Project consists of the following separate contracts: HVAC Work Project No. 47208-H Electrical Work Project No. 47208-E B. The suffix letter at the end of the project number distinguishes the separate Contracts. The Sections in Division 01 of the Specifications which have more than one suffix letter (such as this Section) are common to each related Contract. 1.03 SUBSTANTIAL AND PHYSICAL COMPLETION DATES A. Substantially complete the Work within 209 days after the Agreement is approved by the Comptroller. 1. The time allocated for the performance of work under this contract includes 10 days for notification of the Contractor of the Comptroller's approval of the Agreement. 2. The approval of the Agreement by the Comptroller constitutes the filing of the Contract Documents as a public record and notice to the Contractor that a fully executed contract exists between the Contractor and the State. B. Physically complete the Work within 90 days after the established Substantial Completion date. 1.04 CONTRACT AWARD SUBMITTALS A. Submittal No. 1: Submit the CONTRACTOR'S LIST OF SUBCONTRACTORS-SUPPLIERS information required in SCHEDULES AND RECORDS Article in Specification Section 013000 not later than 15 days after approval of the Contract by the Comptroller. B. All bidders are expected to cooperate in implementing this policy.
