Description | All questions must be submitted in writing, via email, to the POC's listed below, no later than August 26, 2022, no additional questions will be accepted. No site visit will be scheduled for this requirement. POP 16-SEP-2022 TO 30-MAR-2023 Project Address: 6101 Airport Rd, Redding, CA 96002 Project Scope: Asbestos Inspection Dear Mr. McBroome, Guzi-West certified asbestos personnel collected suspect asbestos-containing material samples at the structure referenced above on June 1, 2021. Following sample collection, the samples were submitted under chain-of-custody documentation to a California-certified laboratory for analysis by polarized light microscopy. As evidenced by the attached laboratory report, asbestos was identified in the following materials and locations: o Brown Ceiling Tile Mastic/Workshop Ceiling (2% Chrysotile Asbestos) o Brown Ceiling Tile Mastic/Workshop Ceiling (2% Chrysotile Asbestos) o Beige Cementitious Siding/Exterior Workshop Siding (35% Chrysotile Asbestos) Guzi-West recommends certified asbestos abatement workers remove the asbestos-containing materials from the affected areas and ensure proper disposal. We will follow up with a project invoice within 7 business days. Thank you for the opportunity to work on the project, and feel free to call/e-mail if you have any questions. NOTE: All invoices must reflect the contract order number and invoice number and the final invoice must be marked "FINAL". Contract Payments: The Government shall pay the Contractor upon submission of proper invoices for services delivered and accepted for the portion of the work actually performed under the contract. Payment shall be made as indicated in the contract's Payment Schedule not on percentage of completion. Deliverables as defined in the Payment Schedule of the contract must be accepted by the Government before payment will be made. Payment estimates must be submitted no more often than monthly. Percentages billed must not be calculated beyond two decimal places for each line item on a payment estimate. Each line item must give a detailed description of: The work item being invoiced The negotiated amount The percentage of work completed for the billing period And earnings to date Please refer to the award document for necessary submittals prior to submitting payment estimates. 4.0 WORK TO BE PERFORMED 4.1 Service Scheduling. Within seven (7) days after award of contract, the contractor shall submit to the COR, for approval, a service schedule indicating the time and sequence of work. If unforeseen events cause a variation in the scheduled service, the contractor shall notify the COR in advance of the delay or as soon as possible if unable to notify in advance. Failure to provide the service or variation from the agreed schedule will require written explanation by the contractor. If and when additional service(s) are added, the schedule will be modified. 4.2 Abatement Work Tasks. The specific Asbestos Containing Material (ACM) to be abated is identified in Appendix B - Asbestos Test Results. 4.3 Breakdown of scope of work is as follows: (Appendix A, contractor shall field verify all existing conditions). W9123822Q0034 Page 48 of 67 The workshop repairment consist of removing and replacing ceiling tiles, substrates, wood molding, paint walls and replace anything damaged while repairing the components identified within the Workshop Building for the U.S. Forest Service at Redding, CA. Contractor must coordinate with PSW, through the COR, to clear out spaces for work related with ceiling replacement unless Optional CLINs (see 4.4) are exercised. The following is a breakdown of work to be performed. Contractor is advised to visit site for field verification (3.2 Inspections): REMOVE Remove existing 12" X 12" tile and mastic, repair ceiling substrate as required to install new ceiling tile. Abate asbestos. Remove existing Light Fixtures. Contractor to take care not to damage and store for reinstall. Remove existing Mechanical diffusers and associated hardware. Remove existing smoke detectors and associated hardware. Remove existing wood molding, take care to preserve for re-use. INSTALL Install new 12" x12" non-hazardous acoustical ceiling tile. Pattern to match existing tile. Install per manufacturers recommendations. Re install light fixtures and restore to original working order. Test to ensure reinstalled fixture operations are restored. Re install mechanical diffusers and restore to original working order. Test to ensure re-installed diffusers functions are restored. Re install and exiting smoke detectors and restore to original working order. Test to ensure operation to re-installed smoke detectors are restored. Patch and repair walls damaged by the removal of the ceiling tiles. Patch and repair wall as required and re install molding. Paint walls as required after ceiling tile and other item are installed (all items to match existing). Contractor shall replace any items damaged during construction. |
