Description

Merrimack County is seeking proposals from responsive and experienced vendors to provide a design, procure material, provide demolition, and install a low VOC Polyurethane or Epoxy application to medical, bathroom, and processing areas of Merrimack County Correctional Facility located at 314 Daniel Webster Highway Boscawen, NH. Epoxy Applications - MCDOC Renovation Project The proposed applications must be designed for use in high traffic, institutional or correctional facilities and have no sharp edges or corners in the finished product application. Appropriately remove all of the existing tile and or paint in the project areas. Appropriately prep all project surfaces for new applications. Apply low VOC Polyurethane or Epoxy floor systems, with new cove base, in accordance with manufacturers instructions and which is designed for use in institutional or correctional facilities. Due to current COVID-19 restrictions, proposals will also be accepted via email to the Merrimack County Purchasing Department contact information provided above. All proposals must be signed by an official agent or representative of the company submitting the proposal with authority to bind the company. If the organization submitting a proposal will be relying upon a subcontractor, or will be entering into contract(s) for any portion of the work or product to meet the requirements contained herein, this must be clearly stated in the proposal. Additionally, all costs in the proposal must be "all-inclusive" to include any outsourced or contracted work. All costs must be itemized to include an explanation of all fees and costs.Please use specifications provided in this Request so that proposals can be accurately scored and rated. Please include any added services and/or warranty information along with pricing document(s) for rating purposes. The successful respondent will be required to execute a contract with the County which will incorporate, at a minimum, the Terms and Conditions contained in this Request for Proposal. A copy of the RFP and the successful Proposal will be attached as exhibits to the Contract. Question Deadline 05/02/2023 All questions and inquiries concerning this RFP must be submitted in writing, via electronic mail,to Jonathan Gardner 603) 796-3271 at jgardner@merrimackcounty.net.The County of Merrimack will address all written questions submitted by the deadline and issue an addendum, sent by email to known interested parties,various internet bid news sites,and posted to the Merrimack County web site.It is the prospective proposers responsibility to ensure they have current documents. Warranty: All work must be warrantied. Vendors will offer a minimum 1-year warranty on products used and defects in workmanship. Vendors will honor the offered manufacturer's warranty. Extended warranties on installed products can be offered and detailed as an additional cost option in the proposal response. 5. Invoicing Proposals must have "all-inclusive" pricing that include all applicable fees, materials, and labor charges. Invoicing will be stated as One-time Non-recurring Cost (NRC). Invoicing must be placed on a minimum of Net 30 Terms for the entire order, including FOB destination on any deliveries to 314 Daniel Webster Highway in Boscawen, NH. Vendors will be required to comply with all tax requirements for payment, such as completion of a W-9 prior to payment and tax exemption for the County of Merrimack on applicable charges. Note: All costs and fees must be clearly outlined described in each proposal. 6. Qualifications Respondents should provide the following items as part of their proposal for consideration: A description of applicable experience and any specific experience with Merrimack County. Examples of commercial sites (references) that were successfully installed and/or serviced by your organization. A full product and service satisfaction plan. Any services and/or product information that is applicable to the project outlined. A "Proposed Awardee" will be generated after the evaluation and scoring of all proposals is complete. The highest rated proposal will be presented to the Merrimack County Board of Commissioners for approval and the Award will be issued once the Board of Commissioners has reviewed the proposal and approved the expenditure. The final Award will be issued in the form of an executed award contract.
