Crack Sealing, Rejuvenation, Fog Sealing

All successful bidders shall furnish proof of liability insurance in the amount of not less than $1,000,000.00 per occurrence and $1,000,000.00 aggregate. Successful vendor will be required to provide a performance and payment bond. All successful bidders shall furnish proof of Workers Compensation insurance. The term of all successful bids will be One year (twelve months) March 13, 2023 to March 13, 2024 In addition to the above specifications and bid requirements, the following bid specifications and requirements shall be applicable to each particular bid as hereinafter specified. Bids are hereby requested for crack sealing of cracks 1/8 and larger on county asphalt road surfaces with hot applied rubberized asphalt. All bids shall include all material, equipment, and labor necessary for crack sealing in place. All materials and methods of application of crack sealing materials that are bid shall be applied in accordance with the manufacturers specifications. The bidder shall furnish reasonable and necessary information with respect to crack sealing materials that are bid, including specifications for application. Bidder shall furnish traffic control and signs as required by the County for safety purposes. This bid is submitted as a legal offer and any bid when accepted by the County constitutes a firm contract. Oklahoma laws require each bidder submitting a bid to a county for goods or services to furnish a notarized sworn statement of non-collusion. I, the undersigned, of lawful age, being first duly sworn on oath say that he (she) is the agent authorized by the bidder to submit the above bid. Affiant further states that the bidder has not been a party to any collusion among bidders in restraint of freedom of competition by agreement to bid at a fixed price or to refrain from bidding; of said prospective contract; or in any discussions between bidders and any state official concerning exchange of money or other thing of value for special consideration in the letting of a contract; that the bidder/contractor has not paid, given or donated or agreed to pay, give or donate to any officer or employee of the State of Oklahoma (or other entity) any money or other thing of value, either directly or indirectly in the procuring of the award of a contract pursuant to this bid.
