Description

This is a service / maintenance or supply contract in San Jose, California. Contact the soliciting agency for additional information.The County of Santa Clara (County) is requesting bids from prospective suppliers to provide and deliver methadone medication as required by the Santa Clara County Department of Alcohol & Drug Services Addiction Medicine Division as specified in this Invitation to Bid (ITB). Santa Clara County operates 3 methadone clinics, the Central Valley Clinic located on the Valley Medical Center campus in San Jose, California, South Valley Clinic in San Martin, California and the Alexian Health Clinic in San Jose, California. The three (3) Clinics provide methadone medical for 600 patients per year. The term of this Contract shall be five (5), unless terminated earlier or otherwise amended. The estimated annual quantity is provided based on historical purchase history. County does not guarantee, whether expressed or implied, ordering the exact quantity but reserves the right to purchase more or less quantities. License/Permit to Sell Methadone Oral Concentrate: Any bidder submitting a bid for this requirement shall provide a copy of the companies permit or license authorizing them to sell methadone oral concentrate. Copy of a valid State Narcotic Treatment Program (NTP) License Copy of a valid Drug Enforcement Administration (DEA) License Failure to provide such licenses will result in bidder being deemed non-responsive. Any bidder submitting a bid for this requirement must meet all applicable regulatory standards as established by federal entities for the manufacture, distribution, and sales of federally controlled substances.
