Description

Questions Due Date: Jun 22nd 2022, 5:00 PM MST Tempe Union High School District - Purchasing Department is soliciting formal quotes from all qualified Contractors to provide all necessary equipment, labor, apparatuses and material to perform amphitheater slab repair along with proper disposal of all debris at 1001 E. Knox Road, Tempe, Arizona 85281. The successful Contractor must provide the following services: 4.1. The Contractor shall be responsible for all equipment, parts, materials, apparatuses and all things necessary to complete this project; 4.2. Responsible for removal and disposal of all debris and unused parts, materials and equipment; 4.3. Saw cut the deteriorated concrete slab on grade according to repair plan S3.0; 4.4. Remove the concrete (properly disposed of all concrete); 4.5. Remove four (4") inches of soil and replace with ABC material per original soil report; 4.6. Material and Compaction per soil report; 4.7. Install epoxy dowels in the existing slab (per detail 1 & 2/S3.0.) 4.8. Replace new concrete as specified in General Structural Notes (GSN); 4.9. Repair cracks at steps with elastomeric materials; 4.10. The pricing sheets indicate only probable items intended for procurement. Tempe Union High School District reserves the right to limit the procurement only to the items actually required during the award period; and 4.11. All materials and supplies requested by the Department must be of professional-grade, commercial quality/support along with all any warranty information. 4.12. Special instructions: The Contractor shall confine operations at the site to areas permitted by law, ordinances, permits and the Contract Documents, and shall not unreasonably encumber the site with any materials or equipment. The existing facility will be occupied Monday through Friday from 7AM to 3PM. No work to occur during those days and hours. Contractor must work evenings, nights and weekends to complete the scope of work; and The Contractor will have the option to work during school hours if they can prove that it will disrupt the operation of school and compromise life safety. The District/Owner will still have the final decision regarding if the Contractor can work or not work during school hours.
