Description | The Town of Lamoine seeks quotations for furnishing and placing an estimated 2,850 tons of 12.5mm Hot Mix Asphalt on Maxwell Avenue, Lorimer Road, and Marlboro Beach Road & Needles Eye Road. Shimming of the paving area is to be done as directed by the Town of Lamoine Road Commissioner or his designee. Proposals will be accepted only from contractors pre-qualified by the Maine Department of Transportation for Highway Construction or Paving Projects. The successful Bidder will enter into a paving contract with the Town of Lamoine. The Contractor shall supply a material gradation to the Lamoine Road Commissioner or his designee. Asphalt delivery slips shall be provided for each day paving. The Contractor shall have M.D.O.T. approved Lab., with equipment, and run one asphalt extraction and gradation in the setup. The Contractor will provide a copy of all tests to the Town of Lamoine Road Commissioner or his designee The Contractor shall provide work area signing and sweeping as directed by the Town of Lamoine Road Commissioner or his designee. The Contractor shall provide all the certified flaggers and traffic control personnel as deemed necessary for the project. Coast of flagging, signing and sweeping shall be included in the mix price. Contractor shall grind any and or all joints as directed including paved driveways as directed. The cost of grinding shall be included in the price of the mix. Contractor shall supply all trucking needed to complete this contract. Trucking shall be included in the price of the mix. Contractor shall tack all joints as directed. Also, tack shall be applied between old pavement and shim as directed on all roads and areas to be paved. Work shall be government by the State of Maine Department of Transportation, Standard Specifications, Revisions of November of 2004, and/or updated versions. Paving shall begin no earlier than July 1, 2023 or as weather permits and be completed no later than October 15, 2023. Each bidder must submit with their bid proposal a certified check, bid bond, or cash in the amount of 5% of their bid as the guarantee that the Bidder will enter into a contract. If awarded, said check, bid bond or cash will be returned to all, except the two lowest bidders, immediately following the opening of the bid, and the guarantee will be returned to the two lowest bidders after the Contractor and the town has agreed to the contract. If a contract has not been executed within thirty days after the opening of the bids, the surety will be returned upon the demand of the bidders at any time thereafter, so long as the bidder has not been notified of the acceptance of their bid. The bid bond shall name, as the owner and oblige, the Town of Lamoine. The Contractor shall furnish a Performance Bond, and a Payment Bond for labor and materials in the amount of the total contract price. Each bond shall be on the latest version of A.I.A. Form no. A-311, or on another bond form approved by the Town of Lamoine, shall be accompanied by the companion power of attorney, and shall name, as the owners and obliges, the Town of Lamoine. The Payment Bond shall also include all providers of labor or material as possible claimants and obliges. Any company or firm the Town contract with must provide certificates of insurance for the following types of insurance: a) Workers Compensation Insurance as required by the General Laws of Maine, b) Contractual Liability Insurance with at least a $1,000,000 limit of liability.
