The scope of work consists of reducing the width of Gibbs Street from 4 lanes to 3 lanes, installation of a compact round- about at the intersection of Gibbs Street and Indiana Street, concrete curb and gutter, proposed drainage, ADA compli- ant curb ramps, new sidewalks, concrete drive approaches, traffic control includ- ing signing and pavement markings, and street lighting. Only pre-qualified contractors are eligible to submit bids for this project. Pre-qualification status must be in force at the time of bid, at the time of award, and through the life of the construction contract. For work types that ODOT does not pre-qualify, the LPA must still select a qualified contractor. Subcontractors are not subject to the pre-qualification requirement. The "prime" contractor must perform no less than 30 percent of the total original contract price. Bids will be taken for General and Civil contracts. All bids must be signed and submitted on the blanks which are provided in the contract booklet. The bid guaranty may be of two forms: 1. A Bid Guaranty and Contract Bond using the form in the Contract Docu- ments. (The amount of the bid does NOT have to appear on this form.) 2. A certified check, cashier's check or letter of credit in favor of the City of Maumee, in the amount of 10% of the bid. If the contract is awarded, a Contract Bond will be required, which is a 100% payment and performance bond. After the award of the contract let by competitive bid and prior to the time the contract is entered into, bidders shall submit the affidavit required under the Ohio Revised Code, Section 5719.042 that the bidder was not charged with any delinquent personal property taxes in Lucas County, Ohio. The successfull bidder will be required to pay Davis- Bacon wage rates. A Disadvantaged Business Enterprise (DBE) goal of 7% is required for this proj- ect. The City of Maumee reserves the right to reject any and all bids and to waive the irregularity in any bid and to determine the lowest and best bidder. No bidder may withdraw its bid for a period of 60 days after the scheduled closing time for the receipt of bids.
