Description | The requested services include full length rehabilitation of approximately 367 LF of 78”x96” concrete pipe. A sewer inspection found that there was a large circumferential crack near the 14th Avenue Overflow that resulted in an above ground sink hole near the Great Miami River. Additional horizontal cracks were observed through the length of the pipe as well as severe corrosion. The pipe shall be rehabbed using a cementitious liner. The project completion date will be determined after council approves contract award. No bid will be allowed to be withdrawn after it has been deposited with Public Works & Utilities. Proposals shall be accompanied by a bid guaranty bond in the amount of 10% of the bid or a certified check, cashiers check, or letter of credit in the amount of 10% of the bid. The successful bidder will be required to execute the contract and to provide a 100% performance bond within ten (10) days after the award of the contract. In case the bidder neglects to so execute the contract, the check accompanying the proposal shall be forfeited to the City of Middletown, not as a penalty, but as liquidated damages. The address and telephone number of the bonding company and agent must be included with all bid guaranty bonds and performance bonds. If applicable, bidders must comply with the State Prevailing Wage Rates on Public Improvements in Butler County and the City of Middletown, Ohio as determined by the Ohio Auditor of State Division of Industrial Compliance & Labor. Prior to submitting a proposal, contractors must certify that they have met the requirements of the Ohio Bureau of Workers Compensation's (BWC) Drug Free Workplace Program to be eligible for state funded construction projects including grants within ten days of the bid award. The bid will be deemed non- responsive and not eligible for the award of contracts if this certification is not met. The right to reject any oral bids is reserved by the City of Middletown. Informalities may be waived at the option of the City Council. |
