2022 - West Creek Ditch Maintenance State Road 2 to Singleton Ditch Each Bid Must Be Submitted on Form #96, and the Bidders Shall Execute a Proposal Form and a Non-collusion Affidavit Which Are Part of Form #96. Bids Must Be Submitted in Sealed Envelopes With the Type of Bid Marked on Same. The Contractor Shall Provide and Include an Affidavit That States He/she Has a Valid General Contractor's License in Lake County Together With a Notarized Affidavit That It Will Comply With the County's Affirmative Action Program. All Bids Must Be Made in Full Compliance With the Laws Governing Such Matters. A Bond, Certified Check, or Cashiers Check Equal to 5% of Bid Must Accompany All Bids. One of the Sureties Must Be a Resident of Lake County, Indiana, or a Surety Company Authorized to Do Business in Lake County, Indiana. The Right to Reject Any and All Bids is Reserved.
