Description

The project is located at the City of Jacksonville Beach Well # 16 Construction (Well # 16) Site which is located at the southwest corner of the 8th Ave. N and 11th St. N. intersection and Well # 11 Site (Well # 11) which is located at the intersection of Shetter Ave and 6th St. N, straddling the rear west driveway of 528 Beach Blvd. The Well # 16 scope of work (Work) includes clearing and grubbing the Site, drilling and constructing, testing and placing a new water supply well into service, installing a new vertical turbine pump, discharge piping, valves and appurtenances, instrumentation, constructing a new building around the new water supply well, constructing a new 10-inch and 12-inch ductile iron pipe from the well pump to the existing 12-inch raw water pipe (constructed by others) and connecting the Well # 16 pipe to the existing 12-inch raw water pipe. The Well # 16 Work also includes electrical service to and distribution throughout the Well # 16 building and site, constructing the telemetry system and incorporating the instrumentation and telemetry into the City's existing supervisory control and data acquisition (SCADA) system. Well # 16 sitework includes constructing the new concrete driveway, new vinyl fence and gates, site lighting, landscaping, grading, irrigation system and temporary 6 foot-high chain link security fencing with privacy screening around the Well # 16 Site and staging areas throughout the project duration. The scope of work at the Well # 11 Site includes demolishing and removing all equipment from the Well # 11 Site including, but not limited to mechanical, electrical, telemetry, instrumentation equipment, piping, valves, chain link fence, equipment hatches, utility boxes, concrete pads, gravel and noted vegetation; plugging Well # 11, planting and seeding the Well # 11 site. Please submit triplicate completed Bid Proposal Forms in one sealed envelope clearly marked with bid number and bid name. Place the sealed envelope within another sealed envelope clearly marked in the same manner as the first envelope. This is done to prevent accidental opening of the sealed bids in our mailroom. Bid to be delivered to 1460A Shetter Avenue Opened publicly Each bid must contain either evidence of the Bidder's qualifications to do business in the area where the project is located or covenant to obtain such qualifications prior to award of Contract. To demonstrate his qualifications to perform the Work, each Bidder shall submit with his Bid written evidence as to his financial status, previous successful contractual and technical experience in similar work including references, description, and volume of present commitments, evidence of possession of, or covenant to obtain prior to award of Contract, valid state, county, and local licenses covering all operations and all areas of political jurisdiction involved in the Work of this project and such other data as may be requested by the CITY. No bidder/company will be issued a contract for the work, who in the past seven (7) years has filed for bankruptcy personally or has been an owner/officer or principal of a firm, which has filed bankruptcy in the past seven (7) years. Contractor must be able to exhibit to the City of Jacksonville Beach that he can perform a minimum of 30% of the total contract work with their own labor work force.
