Description

When signing in, be sure to check the project name listed on the sign in sheet to ensure you are recording your attendance correctly. Failure to do so will cause you to be ineligible to submit a quote. There are no exceptions. Due to Covid-19, you must wear a face mask and you may be screened prior to entering the site. Late arrivers at the site visit will not be allowed to participate nor submit a quote for this project. If you are unable to attend, you will not be allowed to schedule a site visit at a later time. This is an RFQ (request for quotes). This is not an IFB (bid). Brooklyn DDSOO intends to award the contract to the most responsible and responsive vendor that provides the lowest cost and meets the requirements detailed in the scope of work. Upon request, vendor must demonstrate relevant experience on prior projects of similar magnitude. Brooklyn DDSOO reserves the right to reject any and all quotes. In deciding whether to or not to submit a quote, please be aware the awarded vendor must adhere to the time frames outlined in the scope of work. The awarded vendor will also be required to submit certificates of Liability Insurance and Worker's Compensation Insurance as well as completed Lobbying Law Forms, and Signed Appendix A. Please be prepared to submit these in a timely fashion. Brooklyn DDSOO will make no concessions to the vendor for any alleged misunderstanding or deception because of quantity, quality, character, or other condition. Brooklyn DDSOO's interpretation of specifications shall be final and binding. Please note all staff working at the project site will be required to sign an attendance sheet for record purposes. This includes the residence visitors log and/or an attendance sheet provided by the maintenance department. NYS Labor law requires that all contracted employees performing maintenance, repairs or capital improvements in state owned buildings receive the prevailing wage and supplements rate for the occupation in which they are working. If this is a public work contract covered by Article 8 of the Labor Law or a building service contract covered by Article 9 thereof, neither Contractor's employees nor the employees of its subcontractors may be required or permitted to work more than the number of hours or days stated in said statutes, except as otherwise provided in the Labor Law and as set forth in prevailing wage and supplement schedules issued by the State Labor Department. Furthermore, contractor must pay at least the prevailing wage rate and pay or provide the prevailing supplements, including the premium rates for overtime pay, as determined by the State Labor Department in accordance with the Labor Law. NYS law requires the vendor to keep detailed records for each person employed including name, address, telephone number, social security number, occupational classification in which worked, hourly wage rate paid, supplements provided, number of hours worked, and actual wages paid. Certified payroll record/s MUST be submitted at the time of invoicing. Effective April 28, 2008, if this is a public work contract covered by Article 8 of the Labor Law, the contractor understands and agrees that the filing of payrolls in a manner consistent with Subdivision 3-a of Section 220 of the Labor Law shall be a condition precedent to payment by the State of any State approved sums due and owing for work done upon the project. Without submittal of certified payroll record/s, payment will not be made. Subcontracting: OPWDD does not allow subcontracting on this type of work. Service-Disabled Veteran-Owned Set Aside: No Minority Owned Sub-Contracting Goal: 30% Women Owned Sub-Contracting Goal: 7% SCOPE OF WORK includes, but is not necessarily limited to the following: -in the rooms that are being painted, move furniture to center of room and cover all furniture -remove all pictures and shelfing from wall that are being painted -repair any holes in walls and prep walls to be ready for paint -Paint all doors and door frames in a semigloss finish -prep and paint all ceilings in a flat finish -after all the prep and painting and cleaning is complete move all furniture, pictures and shelfing back to where they were -prep and paint all walls in a satin finish
