Description

The objective of this project consists of replacing Spot light fixtures and Pathway lighting fixtures in the Flag Pole Area with LED fixtures at the Pawnee Indian Health Center. Contractor shall replace 3 Spotlight and 6 Pathway lighting fixtures in the Flag Pole Area, located at the front of the Pawnee Indian Health Center facility, with the latest technology of Light Emitting Diode (LED) lighting fixtures. Contractor shall use fixtures capable of adjustment in order to match lighting Color Temperature and brightness level of existing outdoor parking area lighting and wall luminaires. The fixtures exterior paint color must match existing fixtures (Green), or be painted with "manufacturer quality" paint of matching color, before installation. Contractor shall run new Branch Circuit Wiring in this area where applicable. This modification is to correct an error in the Description. This is a combined synopsis/solicitation for items prepared in accordance with FAR Part 13. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. The solicitation is issued as Request for Quotes (RFQ) 246-23-Q-0090. This procurement is 100% buy Indian set-aside under NAICS code 238210 with a size standard in millions of dollars $19.0. Contractor shall furnish and install spot light fixtures and pathway lighting in the Flag Pole area at the Pawnee Indian Health Center 1201 Heritage Circle Pawnee, Oklahoma 74058 in accordance with the statement of work. The period of performance for this requirement is 60 calendar days from Notice to Proceed. Questions are due no later than May 12th, 2023 5pm CST. THE INDIAN HEALTH SERVICE SHALL NOT CONSIDER YOUR QUOTE FOR AWARD IF YOU FAIL TO PROVIDE A PROPOSAL CONTAINING: A) Firm, Fixed price quote with attached bid sheet B) Technically specifications of the statement of work, acceptable materials list to be used. C) (2) Relevant past performance of this type of work, shall provide organization's name, point of contact, summary of project and award amount D) Bid bond form (SF24) attached is required at $35,000. If over $35,000 and selected as the acceptable vendor proposal, performance (SF25) and payment (SF25A) bonds shall apply Offers shall be evaluated on an acceptable/non-acceptable basis. Unacceptability of "A" and "B" (stated above) shall render the entire quote unacceptable. Evaluation shall proceed as follows: 1. Quotes shall be ranked by price, from low to high 2. Quotes shall then be evaluated for technical acceptability beginning with the lowest priced quote 3. This process shall proceed until a technical acceptable quote is identified THE INDIAN HEALTH SERVICE SHALL NOT CONSIDER YOUR QUOTE FOR AWARD IF YOU FAIL TO MEET THE REQUIREMENTS IN THE STATEMENT OF WORK CONTRACT AWARD SHALL BE MADE TO THE RESPONSIBLE, RESPONSIVE OFFEROR OFFERING THE LOWEST REASONABLE PRICE AND MEETING ALL TECHNICAL SPECIFICATIONS. The technical evaluation shall be a determination based on information furnished by the vendor in the submitted proposals. The Government is not responsible for locating or securing any information which is not identified in the offer. The Government reserves the right to make an award without discussions.
