Description

The Haworth Board of Education (Haworth Public Schools) hereby advertises for qualifications proposals in accordance with N.J.S.A. 18A:18A-21(a,b) on the following project: Bid No.: 2023-001 Title of Bid: Design-Build Services Request for Qualifications (RFQ) Haworth Public School Additions and Renovations Haworth, New Jersey Qualifications Proposals shall be received by a single firm with undivided responsibility for performance and other requirements. Subject: DESIGN-BUILD SERVICES RFQ HAWORTH PUBLIC SCHOOL ADDITIONS AND RENOVATIONS Qualifications proposals must be sealed and delivered to the Business Office of the Haworth Board of Education on or before date and time indicated below. No proposals shall be received after the time designated in the advertisement. (N.J.S.A. 18A:18A-21(b)). The proposal opening process will be a closed process with proposals being opened and reviewed at a later time by the Board of Education's Technical Review Committee. The Board of Education does not accept electronic (e-mail) submission of proposals. FAXED proposals will not be accepted. The proposal opening process will begin on the above advertised date and time. It is the Proposer's sole responsibility to ensure that its proposal is received at the designated location prior to the deadline. No proposals shall be received after the time designated in the advertisement. (N.J.S.A. 18A:18A-21(b)). Each proposal shall be accompanied by the information listed in the RFQ Checklist. All RFI's (Request for Information) are required to be in writing, via Bidding@rscarchitects.com with the subject line HAWORTH PUBLIC SCHOOLS DESIGN-BUILD SERVICES at least ten (10) business days prior to the scheduled proposal submission date. All proposers are required to comply with the requirements of N.J.S.A. 10:5-31 et. Seq., Affirmative Action Against Discrimination (N.J.A.C. 17:27 et. Seq.) All proposers must submit a photocopy of their Federal Letter of Affirmative Action Plan Approval or a photocopy of their Certificate of Information Report, or a completed Affirmative Action Employee Information Report Compliance with Ch. 33, Laws of 1977. An Initial Project Workforce Report will be required from the successful proposer (Form AA-201). Contractors proposing on this project are to comply with the requirements of the Prevailing Wage Rate Determination pursuant to N.J.S.A. 34:11-56.25. A proposer on a public works project for a Board of Education where the cost of the work exceeds $20,000.00 must first have been qualified by the Department of the Treasury, Division of Property Management and Construction, pursuant to N.J.S.A. 18A:18A-27 through 33, and shall submit with his bid a Prequalification Affidavit, a copy of a valid and active NOTICE OF CLASSIFICATION, a certified copy of a Total Amount of Uncompleted Contracts Form and an Affidavit that subsequent to the latest such statement submitted by him, there has been no material adverse change in his qualification information except as set forth in said Affidavit Corporate proposers are required by law (Chapter 33, Laws of 1977) to submit a list of names and addresses of all stockholders owning 10% or more of their stock. A Non-Collusion Affidavit also must be filed with the proposal. The RFQ package will also include other documents that must be completed and returned with the proposal. Failure to comply with RFQ Instructions and to complete and submit all required forms may be cause for disqualification and rejection of the proposal. All contractors named in this proposal, shall possess a valid Public Works Contractor's Registration Certificate pursuant to N.J.S.A. 34:11-56.48 et. Seq., at the time the proposal is received by the Board of Education. No proposer who is on the List of Debarred, Suspended, or Disqualified Bidders or the List of Individuals Engaged In Prohibited Activities In Iran of the New Jersey State Treasurer, the New Jersey Economic Development Authority, or the Federal government shall be eligible to submit on this project. The Board of Education reserves the right to reject any or all proposals pursuant to N.J.S.A. 18A:18A-18, 18A:18A-2(s),(t), (x), (y), 18A:18A-4(a-c), 18A:18A-22, and to waive any informalities.
