Replace Kitchen Cabinets and Counters

Selected vendor must not be on the NYS DOL Debarred List for the period in which service is to be provided. Any subcontracted vendors indicated must not be on the NYS DOL Debarred List for the period in which service is to be provided. The NYS Debarred List contains individuals and entities that have been debarred for violations of Articles 8 and 9 of the Labor Law, as well as for violations of the Workers' Compensation Law.Any sub-contracted vendors must be approved by OPWDD Fiscal Services prior to services provided. Sub contracted agencies are subject to the same review as the primary contractor including required prevailing wage requirements. OPWDD reserves the right to reject any quote and all quotes. Upon request, vendors must be prepared to demonstrate relevant experience on prior projects of similar magnitude.
