Description | Bid Package - A Rebid Phase 3 New Masonry Veneer of Selected Units At Meadow Wood Apartments Each individual bid package proposal shall be accompanied by a certified check or bank draft, payable to the Charlestown Housing Authority, Charlestown Indiana. U.S. Government Bonds, or a satisfactory Bid Bond executed by the bidder of an acceptable surety in an amount equal to five percent (5%) of the base bid plus all add alternates. Sealed envelope shall list the name and address of the Proposal Bidder on the exterior of the envelope. The successful bidder shall be required to provide a 100% Performance Bond and Labor and Material Payment Bond before startingany work or placing materials on the job. The Bonds shall be received within ten (10) days of receipt of Notice To Proceed. All prices bid, are to be free of all Indiana State Sales Tax, and upon request of supplier, a tax exemption certificate will be executed by the Owner to the successful bidder to cover all items subject to this tax. Bids will be received from contractors for work as shown on the Drawings and hereinafter specified and as set forth on the Contractor's Bid Form(s). Contractors need not bid every bid and every bid package, but Contractors must bid the alternates and unit prices within the bid package they are bidding. (Some bid packages have been divided due to their funding sources that cannot be combined. The work for which Bids are requested includes all bonds, insurance, labor, materials, equipment, and items of materials and services required to provide the proper and complete construction of this project. The work shall be constructed in accordance with this Advertisement For Bids, Drawings and Project Manual as prepared for this project by Edward Revers of Revers Architecture and Design, NCARB certified, licensed to practice architecture in the State of Indiana . Bids shall be executed on Indiana State Board of Accounts Form #96 (2013 edition) with Non-Collusion Affidavit, sworn to as required by the Statutes of Indiana, for all bids of $5,000.00 or more. All other data required by Bid Form 96, Part II and Part III must be submitted as required by Bid Form No. 96. All prices bid, are to be free of all Indiana State Sales Tax, and upon request of supplier, a tax exemption certificate will be executed by the Owner to the successful bidder to cover all items subject to this tax. Attention is called to a request that the Contractor shall make a best efforts goal to achieve a state goal of 10% minority and/or women owned business enterprise participation in this project. Bids may be held by The Housing Authority, City of Charlestown, for a period not to exceed forty five (45) calendar days from the date of the opening of Bids for the purpose of reviewing the Bids and investigating the qualifications of Bidders prior to awarding the Contract. Bidders shall state in HUD Form 5369A, Article 6(d) if a longer acceptance period is offered.
