Description

Eureka Springs New Cafeteria. Summary of construction as follows: Construction of a new +/-10,760s.f Cafeteria Facility. All bid packages must be complete, no exclusions or clarifications to the bid packages will be acknowledged. The Construction Manager and/or Owner reserve the right to waive any formality or to REJECT AND ACCEPT any and all bids per A.C.A 22-9-203 and Act 1193 of 1997 Please not the following: 1. Subcontractor and/or supplier must use the bid form given in the Summary of Work. The bid form must be completely filled out. No exclusions or qualifications will be accepted. 2. All subcontractors and/or suppliers are required to fill out and include with their seal bids, the "Bidder Assurance and Disclosure Form" This form shall include a notarized statement attached to the bid proposal affirming that the bidder has not been involved in any collusion with other bidders or public officials with regard to price or terms of a prospective contract in accordance with Act 2161 of 2005. 3. All subcontractors and suppliers must conform to the Summary of Work. 4. All responsible bids will: a .Be delivered in a sealed envelope (A.C.A 22-9-203) b. Be accompanied by a 5% Bid Bond or cashiers' check in the amount of 5% associated bid (A.C.A 22-9-203 & Act 1193 of 1997) c. Include the premium cost to provide a Performance & Payment Bond 5. Subcontractor and supplier bid proposals exceeding $20,000 must: a. Have a current Arkansas Contractor's License # per Act 142 of 1967, amended by Act 293 of 1969, Act 397 of 1971, Act 546 of 1971 as enacted by the General Assembly of the State of Arkansas. This license number must be filled out on the bid form (A.C.A 17-22-101 & 17-25-101) 6. All mechanical, electrical, and plumbing sub-trades shall hold required trades license(s) for the State of Arkansas (A.C.A 22-9-204) 7. All bid forms must show a dollar for "Trench Safety" measures (A.C.A 22-9-212) 8. As required, bidders must review all alternates, either deductive or additive, in regards to bid documents or as requested on the bid packages specific to the trade being bid. These alternates may be used in assessing the lowest qualified bidder (A.C.A 22-9-203 & Act 1193 of 1997) 9. All General Conditions and Supplementary Conditions are located in the project manual. 10. Refer to Summary of Work section for all trade packages, subcontract and supplier bid forms, sample subcontract and purchase orders, and sample Legacy Construction Management, Inc. Insurance Certificate. Construction Manager/School District shall follow the procurement procedures set forth within A.C.A 19-11-801 & 19-11-805. The school district encourages the participation of small, minority and women owned businesses ( A.C.A 22-9-203) A school district may award a contract to the apparent reasonable low bidder, but the bid must be within 25% of the amount appropriated ( A.C.A 22-9-203)
