Needed Service or Supply: Blackhawk Marina Seawall Repairs Ordering Location and Address: Indian Lake State Park 13156 St Rt 235 N Lakeview, OH 43331 Blackhawk Marina: 11496 State Route 365 Lakeview, OH 43331 Project Background: There are two sections of seawall and 16 seawall posts at Blackhawk Marina at Indian Lake State Park in Logan, County that need repaired. The base bid is #1, the seawall repair. Please include #2, corner repair as an alternate and #3, the repair posts as an alternate. Specifications/Scope of Work 1. Seawall Repair by dock 168: Remove two existing concrete seawall panels. Excavate material and dispose. Drive 6" H beam into lake bottom until solid. Install 5' x 8' x 5" steel reinforced concrete panels in between flanges of H beams. Price to use new seawall panels. Drive steel dead man behind each post until solid. Weld 3/4 " rod from steel H beam to steel dead man. Back fill with gabion and 310 crushed stone. 2. Seawall Repair by corner: Remove two existing concrete seawall panels. Excavate material and dispose. Drive 6" H beam into lake bottom until solid. Install 5' x 8' x 5" steel reinforced concrete panels in between flanges of H beams. Price to use new seawall panels. Drive steel dead man behind each post until solid. Weld 3/4 " rod from steel H beam to steel dead man. Back fill with gabion and 310 crushed stone. 3. Seawall Repair posts: Pull and replace 16 seawall posts driving 12" primer painted steel channel. Bolt to existing dead man rod. Maps and/or Photos: Blackhawk Marina dock 168 and corner Rev. 6/3/22 Seawall repair by dock 168 (The pole has been moved by AES) Seawall repair by the corner Seawall repair to 16 posts that are rotted Rev. 6/3/22 Delivery: Blackhawk Marina is located at 11496 State Route 365 Lakeview, OH 43331 Completion/Delivery Date: must be complete before April 1, 2023 Evaluation. The contract will be awarded to the lowest responsive and responsible bidder as determined by the State. Contract Award. The contract will be awarded to the lowest responsive and responsible bidder by low lot total. Low lot total will be determined by multiplying the unit cost by the estimated usage listed in the bid and then adding each of the totals together to arrive at a total for all items. Failure to bid all items may result in the bidder being deemed not responsive. The awarded supplier will be required to sign an Addendum to Bid Quote or Personal Services Agreement. Example attached.
