Parking Lot Resurfacing at Primary Health Sites

The Primary Health sites included are as follows: Greenville Community Health Center, 348 Main Street, Greenville, PA 16125 Autumn Street Health Center, 99 Autumn Street, Aliquippa, PA 15001 New Castle Primary Care, 1112 South Mill Street, New Castle, PA 16101 It is the intent of the Owner to enter into a single contract for the work. Bids shall include a Bid Security Bond or Certified Check, payable to the Primary Health Network, in an amount of five percent (5%) of the bid, in accordance with Instructions to Bidders, as included in the Contract Documents. Furnish and install the following amounts of square feet of exterior concrete sidewalks, beyond the extent shown on drawings, to be located by the Architect in the field. Comply with typical 5 1/2" thick exterior, reinforced concrete walk on 4" gravel base details as shown on drawings (Detail B3/SP2.1) and materials as specified in Section 033000. Include removal and disposal of existing sidewalk and/or lawn and preparation of subgrade.
