Description | This form shall be submitted regardless of the bid total. Bidders who fail to submit the required disclosure form will be considered non-responsive, and their bid will not be considered for award. The disclosure form shall be submitted in a separate sealed envelope clearly marked "Disclosure Form for the Boating Facility Improvements at Buena Vista County Park" showing the date and time of the disclosure submittal deadline, as well as the Bidders name, and shall be delivered to the location specified above, by or before the disclosure submittal deadline. The Work includes, but is not limited to; demolition of existing asphalt access road and parking, construct concrete launch ramp, install Owner-furnished materials, provide and drive steel piling for docks, construct/install various utilities, construct HMAC paved parking with curbs, construct stormwater treatment structures and construct concrete block retaining wall. It is expected that the successful Bidder shall meet the highest applicable industry standards in providing this scope of Work. Partial funding for this project was through Oregon Department of Fish and Wildlifes Sport Fish Restoration Grant Program in cooperation with the USFWS, under Federal Award number F21AF02010-00. Partial funding was provided by the Oregon State Marine Board Boating Facility Grant Program, investing fees and taxes paid by motorized boaters and Waterway Access Grant Program, investing 10-foot and longer nonmotorized boat permit fees paid by nonmotorized boaters for boating facility improvements. To receive bid documents, a physical address and electronic mail address shall be provided. All proposals shall be accompanied by a bid bond equal to ten percent (10%) of the total bid, to be forfeited to the Owner in the event of failure of the Bidder to execute the contract. A one hundred percent (100%) Performance and Payment Bond will be required to guarantee the faithful performance of the contract. It is required that the surety issuing such bonds be duly authorized and licensed to transact surety business in the state of Oregon. This work is a public work subject to ORS 279C.800 through 279C.870 (Prevailing Wage Rates), but is not subject to the Davis-Bacon Act (40 U.S.C. 276a). The Owner reserves the right to postpone the award for up to 30 days, to delete certain items from the proposal, and to award the contract to the lowest responsible bidder. The Owner also reserves the right to reject any bid not in compliance with all public contracting procedures and requirements or for good cause upon finding by the Owner that it is the public interest to do so.
