Description | Reconstruction of Curbs & Sidewalks on 38th Street from Fairview Street to Brown Street, Brown Street from 37th Street to 38th Street; and Drexel Drive from 16th Street to 18th Street In accordance with the drawings and specifications prepared by the City of Anderson Department of City Engineering. Any Bids received after the designated time will be returned unopened. Bids will then be taken under advisement for review by Community Development Department and City Engineer. Contract Documents including drawings and specifications are on file and available either by email request to Amber Lewis-Lilly, Project Contract Specialist at alewislilly@cityofanderson.com 1. Bid shall be submitted on the Itemized Bid Form. 2. Bid shall be submitted on General Bid Form 96 as prescribed by the Indiana State Board of Accounts. 3. Each bid proposal shall include a financial statement, a statement of experience, proposed plan or plans for performing the public work, and the equipment available for the performance of the public work. 4. Each bid proposal shall include a properly executed non-collusion affidavit as required by the statues of the State of Indiana. 5. Bid Security: Pursuant to Indiana Statute I.C. 36-1-9-6, a bidder shall file a certified check or bid bond with each bid, in an amount equal to ten percent (10%) of the total bid proposal, as evidence of good faith that the successful bidder will execute within ten (10) consecutive calendar days from acceptance of a bid proposal, a contract as set out in the specification. 6. City of Anderson Employment Eligibility Verification. 7. Bidders written plan for a program to test Bidders employees for drugs in accordance with I.C. 4- 13-18. 8. City of Anderson certification of Non-Segregated Facilities. 9. City of Anderson NonDiscrimination Clause. 10. City of Anderson Workforce Breakdown Forms. 11. City of Anderson Affirmative Action Checklist. 12. Ineligible Contractors Form. 13. Conflict of Interest Questionnaire. 14. *Section 3 Intent to Comply (Be advised of the updated regulations effective 07/01/2021). 15. *Section 3 Plan (All updated Section 3 documents are included in bid packet). The Instructions to Bidders contained in the Contract Documents are by this reference, made a part hereof and all Bidders shall be deemed advised of provisions thereof and conditions of the Contract, Specifications and Drawings for this project. Civil Rights: The successful Bidder will be required to comply with Title VI and Title VII of the Civil Rights Act of 1964 and Title VIII of the Civil Rights Act of 1968. Wage Rates: Attention is called to the fact that no less than minimum salaries, wages and applicable other benefits, as established by the United States Department of Labor and included within the contract documents must be paid on this project. Performance Bond and Labor Material Payment Bonds: The successful Bidder will be required to provide a 100% Performance Bond and Labor and Material Payment Bond in accordance with said provisions included within the Contract Documents. *Other Federal Requirements: The successful Bidder will be required to provide a properly executed "Section 3 Worker and Targeted Section 3 Work Certification"; "Section 3 Business Concern Certification"; List of all businesses on Project (Contractor and Subcontractor Activity, HUD form 2516)"; Project Workforce Lists (list of employees working on the project from each company)"; a properly executed "Certification of Bidder Regarding Equal Employment Opportunity:; E.O. 11246; a properly executed "Contractors Certification Concerning Labor Standards and Prevailing Wage Requirements" and a properly executed "Certification of Bidder Regarding Non-Segregated Facilities." Prior to award of this contract, the above-mentioned documents must be on file at Department of Community Development within five (5) working days from time of notification. Bidders failure to produce the above certifications in the allotted time, properly signed and executed, or failure to meet the requirements of said certifications shall bar the bidder from being awarded the contract for this Bid. Bid proposals may be held by the Anderson Board of Public Works for a period not to exceed (45) days from the date of the Bid opening for the purpose of reviewing the Bid Proposals and investigating the qualifications of the Bidder prior to the award of a contract. The Board of Public Works reserves the right to reject any and all Bid proposals or to waive any informalities in quoting to the extent permitted by law. One Hundred Percent (100%) of the funds for the undertaking of this project are provided and available under the Community Development Block Grant Agreement between the City of Anderson, Madison County, Indiana and the United States Department of Housing and Urban Development (HUD).
